Your Opportunity We are seeking aFull Time RNSupervisorfor Home Health. Fleet Landing currently has over 140 healthcare beds including a post-acute unit home health long-term care assisted living and memory care. We are seeking an RN whose primary responsibility is to supervise the care of residents assigned to his/her floor. Follows approved medical procedures in all aspects of resident care.

Key Responsibilities: - Completes the initial assessment within 24-48 hours of client admission and prepare care plan.
- Responsible for the observation assessment nursing diagnosis counsel care and health education of the ill injured or infirm and the maintenance of health prevention of illness of others.
- Explains all procedures treatments medications and clients plan of care to client and family as appropriate.
- Charts progress notes on hard copy or in e-format reviews client status and care provided.
- Provides progress notes to the clients healthcare provider; reports when the clients condition changes or when there is a deviation from the plan of care or at least every 60 days.
- Submits required documentation in a timely manner.
- Directs the activities of clinical care to LPNs CNAs and other ancillary staff
- Acts as coordinator of the health care team to maintain continuum of care with all disciplines.

Qualifications:
Minimum Education Required: Must be Registered Nurse. Must hold a valid license in Florida or a Compact State . Current valid CPR certification required.
Experience: One to three (1-3) ears of nursing experience home care experience preferred. Minimum of one (1) year of administrative and/or supervisory experience responsible to oversee nursing and home health aide delivery of service. Working knowledge of home care regulations. Demonstrated management and leadership expertise preferred.
