Providing Home-Based Disability Support Services Across Connecticut.
Team Members support clients with:
- All aspects of home and community life.
- Ensuring consistent safety.
- Providing a healthy and professional friendship.
- Personal hygiene and dressing.
- Scheduling attending and reporting on medical and therapeutic appointments.
- Shopping cooking and cleaning with the client.
- Organizing consistent exercise socialization and entertainment.
Clients live alone or with family. Clients and care plans are directed and monitored by DSS DMHAS BRS DDS Legal Conservators CCCI SWCAA WCAAA and care management access agencies for daily efficiency.
Requirements:
- Pre-employment drug screen and criminal background check.
- Candidates should have one or two years of experience working with and/or providing for individuals who live at home and have had one of the following: stroke concussions brain injury TBI.
- Present a letter from a person and/or employer verifying experience.
- Car drivers license and insurance.
- High-school and/or GED graduate.
Pay Rate:
Dependent on experience and client needs.
