Risk and Patient Safety Manager

The Manager of Risk Management/Patient Safety Officer for Cayuga Health System is responsible for leading and managing the clinical risk management programs across all CHS entities including our Behavioral Health Service Lines and Long-term Care Facility. The Manager will oversee the coordination of risk management resources that cover the enterprise and establish standards of work to ensure the work product of the department aims to optimize value across all disciplines. In this role the Manager oversees all safety event reports and associated investigations analyses action plans and follow-up in accordance with regulatory requirements and patient safety principles.
The Manager works collaboratively with the Director of Clinical Risk Management and Accreditation Manager of Patient Advocacy and Patient Safety Programs administrators directors managers providers and staff members to develop effective strategies that improve patient safety and mitigate risk.
The position also interfaces with the safety and quality staff in identifying safety and quality opportunities assisting in creating annual risk management safety and quality plans and promoting a safety and quality culture. The Manager will oversee a team of Clinical Risk Managers a Peer Review Specialist and an OMH/OASAS Investigator.
Roles and Responsibilities:
- Risk Management Leadership:
- Directs advises and collaborates with staff across the organization to ensure shared ownership of risk management processes and outcomes.
- Leverages data and sustains knowledge in industry trends that will inform strategies and interventions that amplify the culture of safety of the organization.
- Ensures communication and timely reporting to internal and external customers.
- Risk Assessment and Mitigation:
- Conduct proactive risk assessments to identify potential patient safety threats.
- Utilize tools such as Failure Mode and Effects Analysis (FMEA) and Root Cause Analysis (RCA) to evaluate and address risks.
- Ensure compliance with regulatory requirements related to patient safety such as The Joint Commission CMS and state health departments.
- Transformational Leadership:
- Promote a just culture that encourages reporting of safety concerns without fear of retaliation.
- Serve as the champion for patient safety advocating for system improvements to reduce risks.
- Lead and coordinate patient safety initiatives that align with organizational goals.
- Policy Development/Compliance:
- Create and update policies and procedures that support best practices in patient safety.
- Ensure compliance with accreditation standards and regulatory agencies (e.g. OSHA FDA HIPAA).
- Collaborate with legal and compliance teams to address malpractice risk and patient harm reduction.
- Training and Development:
- Develop and co-lead patient safety training programs for all levels of staff with the Manager of Patient Advocacy and Patient Safety Programs.
- Provide coaching and mentorship to frontline staff on safety best practices.
- Ensure new employees receive orientation on patient safety protocols.
- Data Analytics and Performance Improvement:
- Collect analyze and interpret patient safety data to track performance and outcomes.
- Use evidence-based strategies and performance improvement methodologies such as Lean Six Sigma or PDSA (Plan-Do-Study-Act).
- Report key safety metrics to leadership and participate in quality improvement committees.
- Peer Review and Professional Accountability:
- Oversee and support peer review processes to assess clinical performance and identify safety concerns.
- Ensure peer review activities are conducted in a fair non-punitive manner focusing on learning and system improvements rather than individual blame.
- Utilize peer review findings to develop targeted education training and policy updates that enhance patient safety.
- Maintain compliance with state and federal peer review protections to ensure confidentiality and legal safeguards.
Required Skills and Experience:
- Education: Bachelors degree in a related field required. Education and experience may be considered interchangeable on a year-for-year basis.
- Experience: 7 years of progressive risk accreditation/regulatory and quality management experience including at least 2 years leading complex teams in either a formal or .
- Licensure: Certified Professional in Healthcare Risk Management (CPHRM) and Certified Professional is Patient Safety (CPPS) or must be obtained within 2 years of hire.
Preferred Skills and Experience:
- Understanding of healthcare operations regulatory requirements and the unique challenges of the healthcare environment is highly beneficial
- Ability to lead patient safety initiatives drive a culture of safety and influence change at all levels.
- Excellent verbal and written communication skills for training staff presenting findings and engaging stakeholders.
- Ability to analyze complex safety concerns and implement effective solutions.
- Demonstrated ability to manage multiple and sometimes competing priorities
- Strong analytical skills and experience with data analysis metrics and reporting
- Strong ability to lead investigations into safety events ensuring thorough documentation and action plans.
