Prepares structures and facilitates TEAM meetings to achieve a consensus. Understands and communicates to the TEAM the eligibility guidelines and categories of impairment the difference between IDEA and 504 eligibility and the difference between accommodations and modifications. Guides the TEAM to formulate an Individualized Educational Program (IEP) which provides access to the general curriculum and assists in the development of measurable goals and benchmarks. Uses the data presented at the Annual TEAM meeting to generate the next lEP. Understands the Norwood Public Schools philosophy regarding available resources and communicates protocol regarding placement options. Communicates the empowerment to commit to in-district or out-of-district placements as authorized by the Norwood Public Schools with the understanding: (I) the goals and benchmarks may be achieved in more than one placement (2) the placement options should be observed and agreed upon as preferred by both the sending and receiving parties and (3) the final acceptance is the responsibility of the parent/ guardian. Completes all paperwork according to the regulations and policies of the Norwood Public Schools regarding the TEAM Summary Vision Concerns Service Delivery Grid Summer Services Form and justification for out-of-district or transportation forms. Acts as the community liaison to outside evaluators physicians and mental health resources. Acts as the transition advisor for IEP students entering or exiting services within the Norwood Public Schools. Consults with the appointed Facilitator on a regular basis regarding the needs of the assigned student. Maintains an effective working relationship with parents/ guardians of-students receiving special education services and their advocates. Performs other duties as assigned by the Director of Student Services and/ or Building Principal.
