Heres What You Need
High School Diploma or Equivalent Required
3 Years relevant experience Required (preferably in a healthcare or service environment)
Knowledge of medical terminology preferred
Interpersonal skills and Communication skills
Ability to work with minimal supervision or without supervision when necessary
Proficient computer skills with experience in: MS Outlook MS Office (Word Excel) internet navigation clinically based software and financially based software
What You Will Do
Responsible for coordinating the clerical and administrative processes for entity care transitions management
Responsible for managing departmental mailbox phone lines and/or fax
Supports transition planning activities as needed under the direction of care transition managers: Calls vendors physician offices clinics etc.; Faxes (electronic and traditional) documents as needed
Schedule follow-up appointments call Insurance companies for referral approvals for patients
Assist/support the Case Managers will be assigned to 3 to 4 floors of patients will go into the patients rooms for signatures on documents as well as deliver the Durable Medical Equipment/DME (ie. Wheelchairs walkers wound vacs) to the patients prior to discharge
Makes copies or gathers other data as needed
Collects and enters data as needed
Submits for prior authorizations for transportation and post-acute disposition via insurance portal.
Completes other tasks and projects as assigned
