How to beSuccessful in this Role:
We are looking for someone with case management experiencewho isdiligentdetail oriented and a strong communicator. You will use your exceptional written and verbal communicationskillsfrequentlywithclients family members and community providers to ensure services are being providedin accordance withthe established care plan.We need someone who is prompt with responses and can act with a sense of urgency as well as work well independently and as part of a team.

Throughout the onboarding process you will meet with the ManagersSupervisorsandteams regularly to discuss expectations set goalsidentifypriorities and gain familiarity with current policies procedures and strategic measures. Core Daily Responsibilities:
Responsible for assessment coordination of services and advocacy of olderadultand disabled adult clients.
Screen target populations todetermineif services are.
Provide comprehensive assessment todetermineany individualized client needs.
Provide formal reassessment at specific intervals to gauge progress and continuing needs. Provide care planning which requires decisions about how the identified needs can be met and makeappropriate referrals.
Respond to individuals seeking information andassistanceby providing routine informationreferralandassistance; direct individuals seekingappropriate crisisinterventions.
Developimplementandmonitorservice plans through regular contact with clients and service providers;participatein related case staffing.
Monitor both the progress of the client and the adequacy of the services given.
Maintain client files; prepare documentation correspondence and client-related materials. Produceaccurateinformation for specialized data requestsreportsand reviews; gather information tomaintainand update directory information.
Provide coordination ofappropriate volunteerservices.
Assistsin making presentationsregardingprojects to interested community and professional groups as assigned.
Assess community needs and problems of various client groups.
