Director Care Coordination and Consultation VITALity Catholic Healthcare Services Responsible for the development implementation and ongoing management of Care Coordination Department. Essential Duties and Responsibilities: - Hires and manages appropriate and competent staff to provide this service throughout the 6 counties of the Diocese of Camden.
- Develops and provides educational tools and resources for staff and clients.
- Responsible for interpreting and communicating the Diocesan Care Coordination and Consultation service objectives and strategies throughout the Diocesan community including parishes and other community resources.
- Coordinates the collection and analysis of data and outcomes for Care Coordination services and activities.
- Identifies new opportunities for and develops a network of professional referrals within the community to assist in the delivery of medical and health services as needed.
- Manages the budget for assigned area in a fiscally responsible manner.
- Assists in the creation and distribution of marketing information to elicit interest and expand Care Coordination services.
- Ensures that Care Coordination staff receive appropriate continuing education for professional personal and spiritual growth and promote such growth through personal behavior and example.
- Willing to assist in providing presentations to groups within the community on topics of interest relative to the demographic population being served.
- Contributes to community events as appropriate and requested by Executive Director and VITALity leadership team.
- Responsible for promoting high satisfaction among clients served by Care Coordination team.
- Lead daily activities for a team of RNs and MSWs as they conduct care coordination services throughout the Diocese.
- Oversees staff that supports the VITALity Care Coordination Resource and Referral line and assures synergy of activity between said staff and care coordination team.
- Works with the Executive Director to hire and oversee Diocese of Camden Housing Wellness Nurses and serves as a liaison for this team between Diocesan Housing and VITALity.
- Works collaboratively with other VITALity Ministries to ensure education and awareness of Care Coordination efforts.
- Open to program development and dedicated to working towards ongoing improvement of care delivery processes.
Qualifications and Educational Requirements: - Bachelors degree in Nursing or Social Work. Masters preferred.
- Current RN or LCSW licensure in New Jersey.
- Professional experience required with demonstrated skills and knowledge related to care coordination particularly as it relates to eligibility for services health insurance care transition referral for medical services and outcome evaluation.
- Minimum 5 years clinical experience and minimum 3 years of management experience.
- Excellent communication and interpersonal skills.
- Demonstrated skills in planning organizing and leading professionals in complex functions and processes.
- Understanding of current governmental payment systems and trends along with healthcare options that are available and affordable for the elderly and disabled populations.
- Experience with faith based parish activity preferred.
