Job Duties:
MY CHOICE TRANSITION COORDINATOR /PROGRAM MONITOR
Serves at the OAAS My Choice Transition Coordinator (TC) for the My Choice LA (MCL) project. These positions are work from home positions and are domiciled to assure statewide coverage.
My Choice Louisiana (MCL) is a state program within the Louisiana Department of Health (LDH) and operated jointly by the Office of Aging and Adult Services and the Office of Behavioral Health. MCL grew out of an agreement between the state and the United States Department of Justice (DOJ.) Under the terms of the agreement LDH must seek to transition and divert Medicaid recipients with Serious Mental Illness from unnecessary placement in nursing facilities. MCL staff work with individuals identified as meeting criteria for inclusion in the DOJ specified target population (diagnosis of serious mental illness 18 years of age and older and Medicaid eligible) and assist with desired transition into the community. The assistance provided by MCL staff includes but is not limited to assessment identifying needed supports and services and securing such and intensive transition management. The agreement requires LDH to have the necessary staff to carry out such tasks and the appropriate level of management and supervision to ensure tasks are delivered within a quality and consumer-driven framework. Thus it is critical that OAAS implement the staffing necessary to support required activities.
OCCUPATIONAL SUMMARY:
The TC is responsible for the assessing planning coordinating facilitating and monitoring the transition of individuals from nursing facilities to community settings using guidelines established by OAAS.The TC directly reports to the Program Manager 1-B for their assigned area. The incumbent will coordinate support and track the activities of transition teams consisting of OAAS Regional Office staff Medicaid enrolled support coordination agencies Medicaid managed care organizations (MMCOs) and Medicaid providers in planning and executing transitions. The TC will provide technical assistance facilitate communication between the team and identify and address specific issues preventing individuals from transitioning. The incumbent will participate in policy development and planning to maintain and/or enhance community capacity to support transition targets. This will include combining state and local initiatives and resources to support participants to transition. Incumbent may also participate in activities that involve multiple administrative units within the Louisiana Department of Health and across multiple state agencies.
Incumbent must adhere to pertinent laws rules regulations policies and procedures of LDH OAAS Civil Service and appropriate state/federal licensing and governing bodies. Performs other duties as assigned for smooth operation of the agency.
