The Crisis Diversion Outreach Case Manager will have primary responsibility for face-to-face mental health diversion screenings interventions transitional and intensive case management in addition to specific law enforcement second- responder duties.
Provides outreach services to individuals experiencing acute emotional crises with the goal of diverting the individual from incarceration hospitalization and the criminal court system into the least restrictive setting possible and conducts assessments across systems to determine eligibility for diversion into safe and appropriate treatment options. When appropriate coordinates with Designated Crisis Responders and Mobile Crisis Teams to appropriately utilizes crisis intervention techniques for individual and community stabilization.

Job Requirements:

Physical Requirements:
This position normally requires the physical demands of standing walking bending lifting or performing other work requiring low physical exertion talking and hearing on a regular basis to perform the job requirements. These physical demands are required up to 80% of the time. Position also requires ability to drive with adequate vision.

Employment is conditional upon:
Being cleared by criminal background check and fingerprinting when required
Wage:$23.32 - $28.62 per hour depending on experience
Bonus: $5000 sign on bonus paid in 2 installments for a total service commitment of 27 months
Schedule:Tuesday - Friday 7am to 6pm
