Case Manager - Diversion Outreach Team

Position Summary:
The Crisis Diversion Outreach Case Manager will have primary responsibility for face-to-face mental health diversion screenings interventions transitional and intensive case management in addition to specific law enforcement second- responder duties.

Provides outreach services to individuals experiencing acute emotional crises with the goal of diverting the individual from incarceration hospitalization and the criminal court system into the least restrictive setting possible and conducts assessments across systems to determine eligibility for diversion into safe and appropriate treatment options. When appropriate coordinates with Designated Crisis Responders and Mobile Crisis Teams to appropriately utilizes crisis intervention techniques for individual and community stabilization.

Responsibilities:

Law Enforcement / Jail Diversion

• Acts as Liaison between law enforcement agencies Jail Administrator and Mental Health Professional and City/County Prosecutors regarding recommendations for jail diversion services for an identified individual.
• Responds to law enforcement referrals or co-responds with law enforcement to behavioral health crisis calls and assesses the need to offer and integrate into community support
• Provides limited intensive case management to law enforcement referrals to connect individuals to community supports to meet their basic needs and assess for ongoing needs

Community Liaison

• Act as liaison to behavioral health and other community social and support services for the continued care of individuals for both law enforcement referrals and Trueblood/Diversion Clients

Diversion Agreement Monitoring

• Monitors Trueblood Class Members compliance on monthly compliance reports and provides updates to Diversion Program Manager Defense Attorney and Prosecuting Attorney

Data Collection and Reporting

• Reviews the jail roster daily for referrals and active Trueblood Class Members
• Communicate both Law Enforcement Referrals and Trueblood Class Member needs daily to the Diversion Team.
• Attend daily team huddles to coordinate care and staff cases as needed
• Attend necessary community and provider meetings to coordinate care on behalf of Diversion clients and Law Enforcement referrals
• Regularly report and update treatment providers regarding compliance and communicating Trueblood Class Member needs and status
• Monitor Diversion Agreement compliance
• Identify Jail Diversion options and track reporting data in collaboration with Jail Mental Health Professional Crisis Intervention Team and CC Program Evaluation.
• Provide at minimum monthly updates on diversion activity to Crisis Lead Manager defense attorney and prosecuting attorneys
• Provide transitional and intensive case management for Trueblood Class Members and Law Enforcement Referrals

Jail Services Screening

• Upon receipt of a Diversion referral for incarcerated individuals conduct Diversion screenings at the jail to determine eligibility for Diversion
• Coordinate with necessary community partners internal programs and Jail staff to support individuals while incarcerated and support to prepare for community re-entry
• Refer all screened clients to appropriate and requested community resource for community re-entry support
• Update and coordinate re-entry plan for all eligible Trueblood Diversion Class Members
• Completes necessary paperwork as required meeting established timelines and making authorized entries in the medical record
• Follows all procedures for release of information and maintains complete and accurate concise records and reports
• Works to understand others on the team and is a cooperative team member who willingly helps others perform related jobs
• Maintains confidentiality in accordance with HIPAA regulations and RCW 70.02.230 and educates individuals on matters requiring informed consent.
• Participates in supervision with Crisis Lead Manager and multi-disciplinary team staff meetings training specialist and psychiatric consultation as indicated/requested and required by supervisor.

Less Restrictive Setting

• Supports use of engagement and creative problem-solving with individuals and their families in order to facilitate the least restrictive placement that meets the consumers and communitys needs.
• Promotes and teaches advocacy skills Motivational Interviewing and trauma informed skills to clients
• Perform other duties as assigned

Physical Requirements:

This position normally requires the physical demands of standing walking bending lifting or performing other work requiring low physical exertion talking and hearing on a regular basis to perform the job requirements. These physical demands are required up to 80% of the time. Position also requires ability to drive with adequate vision.

Non-Physical Requirements:

Education

• Associate degree in social services psychology or a related field preferred.
• High School Diploma or GED required with two additional years of relevant work experience considered in lieu of an associate degree.

Experience

• Minimum of one year of experience providing direct client care in a social service setting
• 1 year of experience in intensive case management and/or jail diversion services preferred but not required

Special Skills and Abilities Required

• Knowledge of behavioral health care delivery
• Knowledge of Motivational Interviewing and Trauma Informed Care
• Possess good verbal and written communication skills for communicating with persons served of all ages and staff
• Ability to work beyond 40 hours when needed as part of salaried exempt managerial position

Licensure Registration Certification

• Agency Affiliated Counselor Registration required and actively working toward completion of degree or other Case Manager Training / Certification
• Valid Washington State drivers license required minimum liability insurance for WA State
• Must be deemed insurable as determined by Catholic Charities liability insurance provider

Employment is conditional upon:

Being cleared by criminal background check and fingerprinting when required

Wage:$23.32 - $28.62 per hour depending on experience
Bonus: $5000 sign on bonus paid in 2 installments for a total service commitment of 27 months
Schedule:Tuesday - Friday 7am to 6pm
