Development and Community Outreach Coordinator
PAI is seeking a dynamic and goal-driven relationship builder to develop connections that generate qualified referrals for our Employment Services Individualized Home Supports and Respite and Day Support Services. This role involves outreach to case managers families employers schools and other community partners to expand awareness of our services and drive new program enrollments.
The ideal candidate is outgoing knowledgeable about Minnesota waiver programs for people with intellectual or developmental disabilities and comfortable working in both professional and community-based environments.

Key Responsibilities:
- Generate and qualify leads for:
- Employment Exploration Development and Support Services
- Individualized Home Supports (with and without training) and Respite Services
- Day Support Services
- Build relationships with:
- Case managers school transition teams social workers support coordinators and groups and places where families who have a loved one with a disability connect
- Local employers (directly and through chambers and other groups) vocational rehabilitation counselors and community-based partners
- Families and guardians of individuals eligible for waiver services
- Conduct presentations info sessions and one-on-one meetings to explain service offerings
- Represent the organization at career fairs resource fairs chamber events and networking meetings
- Maintain an active referral pipeline and ensure seamless handoff to intake/enrollment staff
- Track outreach activities contacts and results
- Collaborate closely with communications and program staff to align messaging and outreach materials
- Stay up-to-date with DHS guidelines HCBS waiver updates and relevant changes in Minnesota disability services
Preferred Qualifications:
- Experience in sales outreach community relations case management or disability services
- Familiarity with Minnesota Medicaid waiver programs including DD CADI and BI waivers
- Knowledgeable of Vocational Rehabilitation Services (VRS) and employment supports for people with disabilities
- Excellent communication relationship-building and organizational skills
- Ability to manage multiple leads and follow through with consistent outreach
- Willingness to travel locally for events meetings and presentations (mileage reimbursed)
- Valid drivers license and reliable transportation required
- Prior work in HCBS waiver-funded programs or with DEED/VRS preferred
Compensation and Benefits:
