Job Description
Case Manager Supervisor
- Salary Range: $$58011 - $81224/yr based on relevant years of experience
- Location: Larimer county Colorado
- Status: (example full time 40 hours per week)
- Hours: Monday - Friday
Role Overview:
Responsible for overseeing and supporting a team of case managers to ensure high-quality client-centered service delivery. This role provides supervision coaching and accountability to staff while also serving as a resource for resolving complex client needs and system navigation. The Supervisor ensures adherence to agency policies program standards and regulatory requirements. In addition to supervisory responsibilities the Case Manager Supervisor may carry a reduced caseload and provide direct services when needed. This position will demonstrate commitment and provide leadership for Summitstone Health Partners values by delivering visionary and operational leadership while fostering trust collaboration and a diverse and inclusive environment.
Essential Duties:
- Provide direct supervision through regular individual and team meetings performance feedback and development planning.
- Monitor the quality timeliness and compliance of case management services including adherence to documentation standards and regulatory requirements.
- Oversee onboarding training and ongoing professional development for case management staff.
- Serve as a point of escalation and support for high-risk or complex client situations.
- Lead team meetings case consultations and maintain a collaborative and supportive work environment.
- Maintain a reduced caseload and conduct community-based visits to support client and team needs.
- Coordinate with internal departments and external partners to meet client needs and improve service delivery.
- Contribute to program planning goal setting and performance reporting.
- Act as a liaison in cross-departmental initiatives and maintain professional relationships with community partners.
- Perform other duties as assigned.
Required:
- Bachelors degree in Social Work Psychology Human Services or related field required
- More than three years of experience in case management required case management experience in a behavioral health setting strongly preferred
- Equivalent combination of education and experience may be evaluated and considered
Highly Desired:
- Ability to recognize the importance of safety in the workplace follow safety rules practice safe work habits utilize appropriate safety equipment and report unsafe conditions to the appropriate supervisor.
- CAS preferred.
- Bilingual/bi-cultural preferred
