Job Description
Position Details:
- Hours: 37.5 hour work week; Monday-Friday 8am-4pm; with a rotational on-call schedule.
- Salary: $20.51/hr.
- Conduct outreach in areas frequented by homeless individuals to identify potential participants.
- Complete non-clinical assessments and identify barriers to health care access.
- Facilitate enrollment in VA and community health care including behavioral and mental health services.
- Coordinate appointments and communicate with health care teams.
- Collaborate with VA community providers and internal teams to ensure participants receive needed support.
- Educate Veterans on their rights and available benefits.
- Document services and track participant progress in accordance with program guidelines.
- Support housing appropriateness through health care coordination and accessibility inspections.
- Promote long-term wellness by ensuring access to food hygiene and recovery resources.
- Assist with benefit applications and develop exit plans for continued care.
- Bachelors Degree (Masters Preferred)in Social Work with two years experience or Related Degree with equivalent expertise in Health Care Navigation.
- In-depth knowledge and understanding of VA and Mainstream health care services to include behavioral and mental health health and wellness systems of care obtaining access to benefits and other supportive services.
- Ability to conduct outreach case management program and organizational activities during normal working hours some evening hours and occasional weekends as required.
- Valid drivers license clean driving history and reliable personal vehicle. Proof of insurability may also be required.
- Current FBI criminal and child abuse clearance or the ability and willingness to obtain.
