What Youll Do:
Provide in-home primary care visits for seniors in private homes assisted living and skilled nursing facilities.
Complete comprehensive assessments diagnose and manage chronic conditions and develop personalized care plans.
Collaborate with care teams to deliver holistic patient-centered care.
Build trust and rapport with patients and their families through strong communication and a warm compassionate approach.
Qualifications:
Active and unrestrictedNP or PA licensein California.
Experience in geriatrics primary care or home-based care preferred.
A heart for seniors and a drive to provide accessible relationship-based care.
