What Youll Do in This Role:
We are seeking a compassionate and dependable Home Health Concierge to join our Program of All-Inclusive Care for the Elderly (PACE) team. This non-certified position focuses on delivering individualized participant-centered non-clinical support that promotes safety comfort and dignity. Services are provided in participants homes or in contracted settings such as Adult Foster Care Assisted Living or Skilled Nursing Facilities
Primary Responsibilities:
- Provide non-clinical assistance such as companionship light housekeeping errands meal preparation and other supportive tasks as outlined by the Interdisciplinary Team (IDT).
- Support the participants well-being and help maintain a safe and stable environment across various care settings.
- Build trusting respectful relationships with participants and respond empathetically to their needs.
- Communicate observations or concerns to appropriate team members to support participant safety and care planning.
- 1:1 participant
- Work across multiple care settings based on participant needs and care plans.
- Be an important extension of the care team even without a clinical license.

What Youll Need:
- High school diploma or GED required.
- Reliable transportation and a valid Michigan drivers license.
- At least one year of experience working with the frail elderly or in long-term care settings.
- Possess current CPR certification or willing and able to obtain as offered by LifeCircles.
