Job Description

THE POSITION

Are you interested in a career in serving the older adults of Schuylkill County If so consider joining the Schuylkill County Area Agency on Aging (AAA) as an Aging Care Manager 2. DESCRIPTION OF WORK

As an Aging Care Manager 2 you will receive case assignments involving general casework intake and assessment. Your work with older adults will be ongoing from initial contact with individuals their families and professionals through care management responsibilities. Examples of duties include providing assistance to consumers with completion of program forms and making referrals of appropriate community services. You will have the opportunity to regularly coordinate services with physicians community agencies financial institutions hospitals nursing homes and personal care homes. Work involves responding to client emergencies and protective services. Our ideal candidate will manage multiple tasks and display excellent attention to detail skills. You will be required to complete paperwork in a timely manner meeting all deadlines and input confidential client information into state databases.


Work Schedule and Additional Information:

  • Full-time employment 37.5 hours per week
  • Work hours are 8:30 AM to 4:30 PM Monday - Friday with 30-minute lunch.
  • On-call hours when required after training. Some out of county overnight travel may be required.
  • Telework:You will not have the option to telework in this position.
  • Salary:In some cases the starting salary may be non-negotiable.

REQUIRED EXPERIENCE TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelors degree in social sciences behavioral sciences human services or a closely related field; or
  • An equivalent combination of experience and training.

  • You must pass a background investigation.
  • A conditional offer of employment will require a medical examination and drug screening.
  • This position falls under the provisions of the Older Adult Protective Services Act.
    • Under the Act a conditional offer of employment will require submission and approval of satisfactory criminal history reports including but not limited to PA State Police and FBI clearance.
  • This position falls under the provisions of the Child Protective Services Law.
    • Under the Law a conditional offer of employment will require submission and approval of satisfactory criminal history reports including but not limited to PA State Police clearance PA Child Abuse history clearance and FBI Fingerprint clearance.
