Schedule: Full-Time Travel Within Local Area

Key Responsibilities

Collaborate with the healthcare team to develop and implement patient care plans

Provide in-home care and support to patients in Harrisburg and surrounding areas

Ensure accurate and timely electronic charting using EMR systems (TruChart or similar)

Demonstrate strong self-management and scheduling skills

Use mobile technology confidently (tablet mobile phone etc.) to support patient care and documentation
What Youll Bring
-
Valid LPN license in the state of Pennsylvania
-
Associate degree or higher in nursing or a related field

1 years of experience with EMRs (TruChart preferred) or general LPN duties
-
Current AHA BLS and/or ACLS certification
-
Valid drivers license insured vehicle and reliable transportation

Comfortable working independently and adapting to a mobile care environment
