The Department is looking for individuals who are resourceful self-directed energetic and compassionate to provide essential social services to our patients. In this crucial role you will function as a Social Worker 1 in the Continued Care Service of Clarks Summit State Hospital. Work involves complete responsibility for providing and participating in social service casework and the community support planning processes that support and promote recovery and transition from the hospital. This position requires a working knowledge of all Social Service policies and procedures hospital policies and procedures CMS regulations current mental health legislation and resources available to assist patients with day-to-day coping skills and ultimate discharge planning. You will have the opportunity to participate in weekly conferences with the supervisor where ongoing work is reviewed discussed and problem cases handled.
