Responsibilities

Specific goals include:
Outreach and building relationships with faith-based community churches
Identifying a champion at each site and training each champion in the Diabetes Empowerment Education Program (DEEP) curriculum
Empower identified champions to lead DEEP workshops in their communities
Program goal: increase awareness prevention and education to African American Hispanic Native American Asian and Pacific Islander populations who are at risk of life-threatening kidney disease
**This position pays $23.00**

Qualifications
Minimum:
- High School Diploma
- Experience working in the community.
- Experience with Community Organizations and Home Health follow-up
