Job Description

THE POSITION

Perry County Area Agency on Aging is seeking a compassionate Aging Case Aide 2 to provide outreach services information and referrals to consumers person centered counseling and general support to care management. DESCRIPTION OF WORK

In this important role you will provide information and referrals to older adults regarding available support and services. This includes explaining benefits guidelines and application procedures assisting with completing forms and applications making referrals to agencies professionals and other service providers and contacting service providers on behalf of consumers. An Aging Case Aide 2 provides direct one-on-one Medicare counseling to beneficiaries to determine individual needs. Key to this position is recognizing the need for protective and supportive services for abused exploited or neglected older adults and completing a Report of Need assessments in the statewide database. You will have the opportunity to organize public education and outreach events.

Additional duties to include but not limited to:

  • Assisting the senior community center liaison with data entry and management in CoPilot
  • Using the statewide database to complete simple assessments enter documentation and review data for compliance
  • Providing assistance to Aging Care Managers within the department

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:00 AM to 4:00 PM Monday - Friday with 60-minute lunch.
  • Telework:You will not have the option to telework in this position.
  • Salary:In some cases the starting salary may be non-negotiable.

REQUIRED EXPERIENCE TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • One year as an Aging Case Aide 1; or
  • Two years of public contact work including one year in a human services program; or
  • Any equivalent combination of experience and training.

Legal Requirements:

  • This position falls under the provisions of the Older Adult Protective Services Act.
    • Under the Act a conditional offer of employment will require submission and approval of satisfactory criminal history reports including but not limited to PA State Police and FBI clearance.
