  • 33366 per annum (pro rata for part time)

Your role and responsibilities

As the operational manager of the Home First Service within a locality you will play a pivotal role in ensuring that our service is delivered in alignment with Short Term Reablement Plans (STEP) relevant policies and procedures service specifications and quality standards. Your contributions will be essential in the assessment and reablement process.

You will

  • develop reablement plans with customers during initial assessments and subsequent reviews recording these on the customers STEP (Short Term Reablement Plan)
  • assess and adjust the content of the reablement plan as needed recommending the cessation of the plan when appropriate
  • ensure the service is delivered according to the agreed reablement plan with ongoing reviews to achieve the jointly agreed goals and outcomes
  • use IT systems to update customer data efficiently
  • manage the relationship with Business Support effectively ensuring efficient and effective support for the Home First service
  • liaise with colleagues to ensure the effective implementation of reablement plans identifying timely alternative provisions and ensuring customer-focused services
  • provide professional supervision and staff observations to HFPs keeping them informed of changes to policy and procedures including Health and Safety
  • ensure compliance with Health and Safety policies including undertaking risk assessments and workplace handling
  • maintain compliance with regulated training and staff development
  • in difficult or emergency situations undertake direct care work or assist and support wider team members.

The team

Home First provides a short-term reablement care and support service to individuals living in their own homes. This service is designed to assist people following a hospital stay an illness or crisis at home or to help prevent hospital admissions. You will collaborate closely with colleagues from ASC Cluster Teams Independent Providers Health Boroughs and Districts the Multi-Agency Safeguarding team and a diverse range of agencies including those in the voluntary sector.
