Your role and responsibilities
As the operational manager of the Home First Service within a locality you will play a pivotal role in ensuring that our service is delivered in alignment with Short Term Reablement Plans (STEP) relevant policies and procedures service specifications and quality standards. Your contributions will be essential in the assessment and reablement process.

The team
Home First provides a short-term reablement care and support service to individuals living in their own homes. This service is designed to assist people following a hospital stay an illness or crisis at home or to help prevent hospital admissions. You will collaborate closely with colleagues from ASC Cluster Teams Independent Providers Health Boroughs and Districts the Multi-Agency Safeguarding team and a diverse range of agencies including those in the voluntary sector.
