Brief Description of Duties

The position of Case Manager (UDC PATHS PROGRAM) consists of services that help District residents who receive Temporary Aid to Needy Families (TANF) benefits. The incumbent is responsible for coaching and coordinating needed educational medical social and other services for the participant. The incumbent oversees the management of an assigned caseload that is directly linked to appropriate services. Case management includes: (1) assessment of the eligible customer to determine service needs; (2) development of an Individualized Responsibility Plan (IRP); (3) referral and related activities to help the customer obtain needed services; (4) monitoring and follow-up; and (5) evaluation. Case Management does not include direct delivery of medical clinical or other direct services; it primarily involves case management coaching participant advocacy and referral to services to mitigate barriers identified during assessments and interaction with participants.

Essential Duties and Responsibilities

To perform this job successfully an individual must be able to perform each essential duty at a level of satisfaction that meets the terms and conditions of the Memorandum of Agreement (MOA) between the District of Columbias Department of Human Services (DHS) Economic Security Administration (ESA) and University of the District of Columbia (UDC) School of Business and Public Administration (SBPA).

Duties and responsibilities include but not limited to the following:

  • Initiates paperwork with participants referred to UDC PATHS Program.
  • Conducts outreach to individuals in the community and enrolls participants in specific programs as appropriate;
  • Performs and/or recommends a variety of case management services to participants;
  • Consistently exercises discretion and judgment to analyze interpret and ultimately decide what actions are necessary based on the various facts and circumstances of each individual case.
  • Works with supervisory assistance from the PATHS Program Manager and PATHS Principal Investigator in evaluating possible courses of conduct and making appropriate decisions that best meet the needs of the individuals being served.
  • Maintains an active caseload providing interventions as needed and within area of expertise and limits of credentials.
  • Writes a detailed Individual Responsibility Plan (IRP)
  • Works with the individual daily using professional judgment and discretion to implement the team determined IRP;
  • Assists in development implementation and revision of IRPs; assures that services provided are specified in the IRP and monitors progress toward IRP goals;
  • Consults and cooperates with community-based organizations to facilitate referrals for crisis management childcare vouchers and follow up on attaining IRP goals;
  • Maintains participant and program records in accordance with applicable standards regulations and grant requirements.
  • Maintains an elevated level of ethical conduct regarding confidentiality dual-relationships and professional stature.
  • Attends staff meetings and meets regularly with supervisor and other team members to exchange pertinent information and receive supervision as appropriate;
  • Completes required training and ongoing certification as needed;
  • Identifies appropriate volunteer and paid work placements for participants.
  • Responsible for all required reports and correspondence maintenance and security of participants records for auditing purposes.
  • Documents all case notes and prepares and maintains accurate and complete records and completes reports required for documentation of services in the CATCH system.
  • Models professional behavior for participants staff colleagues and agency participants.
  • Remains abreast of best practices in areas of human service delivery and work readiness.
  • Performs the following case management services to PATHS customers such as:
    • Conduct outreach comprehensive assessments including but not limited to basic skills and career assessment
    • Develop and update customized IRP for participants assigned to case load
    • Build a community resources network for PATHS direct referrals for engagement/reengagement and removing barriers.
    • Provide academic and financial aid counseling for participants on case load planning to attend academic programs at UDC and universities or colleges.
    • Advise participants on upcoming work readiness and employment opportunities.
    • Conduct an orientation for new participants to the UDC PATHS program.
    • Develop working relationships with participants.
    • Coordinate multiple activities outlined in the participants IRP and support individuals in all aspects of program enrollment.
  • Performs other duties as assigned.

  • Keep supervisors apprised of challenging situations/ circumstances.
  • Complete work in an accurately and timely manner.
  • Adhere to University guidelines policies and procedures.


Minimum Job Requirements

  • Bachelors degree in Vocational Rehabilitation Social Work Sociology Child Development Substance Abuse Mental Health Human Services Education Criminology Counseling and Therapy or related field(s).
  • Knowledge and skills in community based behavioral health care and evidence of ongoing training and education in related areas such as mental illness substance abuse and/or homelessness.
  • A Masters degree is preferred.
  • Mandatory that the incumbent must have worked within adult training programs.
  • SOAR training is a plus.
  • Strong interpersonal and communication skills.
  • Complete and maintain a current Mandated Reporter training certification.

Condition of Employment:
This is a sponsored program appointment. The duration of this appointment is subject to grant fund availability.
