Brief Description of Duties
The position of Case Manager (UDC PATHS PROGRAM) consists of services that help District residents who receive Temporary Aid to Needy Families (TANF) benefits. The incumbent is responsible for coaching and coordinating needed educational medical social and other services for the participant. The incumbent oversees the management of an assigned caseload that is directly linked to appropriate services. Case management includes: (1) assessment of the eligible customer to determine service needs; (2) development of an Individualized Responsibility Plan (IRP); (3) referral and related activities to help the customer obtain needed services; (4) monitoring and follow-up; and (5) evaluation. Case Management does not include direct delivery of medical clinical or other direct services; it primarily involves case management coaching participant advocacy and referral to services to mitigate barriers identified during assessments and interaction with participants.
Essential Duties and Responsibilities
To perform this job successfully an individual must be able to perform each essential duty at a level of satisfaction that meets the terms and conditions of the Memorandum of Agreement (MOA) between the District of Columbias Department of Human Services (DHS) Economic Security Administration (ESA) and University of the District of Columbia (UDC) School of Business and Public Administration (SBPA).

Condition of Employment: This is a sponsored program appointment. The duration of this appointment is subject to grant fund availability.
Collective Bargaining Unit (Non-Union): This position is not in the collective bargaining unit.
