Job Description

Adelphi is seeking a Licensed Physical Therapist (PT) to provide school-based physical therapy services for students with Individualized Education Plans (IEPs) 504 Plans and other referrals. The ideal candidate will work across multiple school sites in a collaborative educational environment to support students functional independence and mobility.

Requirements:

Current New Hampshire license as a Physical Therapist.

Responsibilities:

Any other duties as direct physical therapy services in accordance with IEP and 504 Plan goals.

Any other duties as evaluations and re-evaluations to assess student needs and progress.

Any other duties as in IEP/504 team meetings and contribute to development of therapy goals.

Any other duties as consultative services to educators families and support staff.

Any other duties as clear and thorough documentation including service logs reports and billing records.

Any other duties as to student scheduling and make-up sessions due to cancellations or inclement weather.

Any other duties as legal proceedings with documentation and expert testimony if required.

Any other duties as with FERPA school district policies and confidentiality standards.

Any other duties as therapy equipment and communicate any material needs to the District.

Any other duties as assigned.
