General Summary:
Develop monitor and adjusts family service plan and individualized client services: Working under the general supervision of a program supervisor independently arranges the sequence of work obtains necessary information and data selects appropriate methods and procedures and varies these as necessary to address different case situations
Provides general client services and case management: offers significant support and guidance as well as service assistance which directly impact the social physical and/or economic well-being of the clients served and at times their families/significant others.
Fluency in Spanish and English required.
Principal Responsibilities and Duties:
Client Referrals:
- Accepts referrals for social services
- Assists clients to assess their situation define their problems and identify acceptable courses of action by availing themselves of recommended community services.
- Completes initial intakes/assessments
- Consults with county/community/agencies to identify and develop resources for client assistance
Family Service Plan and Individualized Client Services:
- Analyze the subjects or issues involved in each assignment or case to develop family service plans for clients family
- Determine intervention strategy and appropriate actions utilizing clients individual needs
- Makes follow-through contacts with clients and other agencies to ensure that needed services have been provided
- Provide care management services to parents/families with economic and/or social needs such as housing healthcare employment assistance food clothing education and other services. For certain program assignments will provide other services with a focus on recently arrived migrant and asylum-seeking youth families.
- Prepares and updates case records/client status reports.
General Client Services:
- Outreach and network with youth families communities schools and other community agencies
- Provides on-going social services (appropriate assistance during crises may provide supportive counseling food clothing housing employment transportation financial assistance)
- Assists families with applications for health insurance offers information and referrals for community members school staff non-school families etc.
- Serves as intermediary between clients and other agencies and supports and advocates for their clients with public and private agencies and individual service providers.
- Provides translation and interpretation services for clients when available
- Co-leads support groups*
- Collaborate with community school coordinator/aide therapist school staff and community partners in providing comprehensive care.
Case Management:
- Ensures recordkeeping-intakes/family needs assessments; family service plans contact notes electronic database entry
- Keeps abreast of changes to and the issuance of new rules regulations policies and procedures pertaining to program eligibility and service delivery.
- Reviews programmatic and personal documents creates and maintains case notes and records drafts brief memos and letters and performs related functions.
- Conduct home visits in accordance with contract requirements when needed
Training & Meetings:
- Attends LTL trainings/other events and regular supervision
- Meets with Linkages team members regularly for the purpose of sharing information and training and to discuss/recommend resolution for problems encountered in the work/case management.
- Attends meetings with the client and school when appropriate
- Attends Sheppard Pratt meetings /activities
Knowledge Skills and Abilities Required:
- Bachelors degree in related field. Equivalent combination of education and work experience may be substituted.
- Prior experience serving parents/families in need (internship experience considered)
- Excellent collaboration skills
- Must be able to demonstrate written and verbal fluency in English and Spanish.
- Experience working within collaborative school-based models of service delivery
- Prior experience/equivalent knowledge of immigrant populations & acculturation issues
- Thorough knowledge of community resources in Montgomery County area.

The pay range for this position is $48214.40 minimum to $53560 maximum. Pay for this position is determined on a number of factors including but not limited to years and level of related experience.
