General Summary:
- In collaboration with the patients interdisciplinary treatment team performs a full range of aftercare and discharge planning services geared to mentally ill patient population.
- Maintains comprehensive knowledge base of agencies organizations and resources within the referral community for discharge planning and placement.
- Communicates with patients based on an understanding of the developmental needs of the population.
- Develops and assists in the implementation of discharge and aftercare plans; works with patients and families to facilitate the community transition and assure appropriate match to patients needs.
Knowledge Skills and Abilities Required:
- Work requires knowledge of psychiatric disorders diagnoses and therapies and basic computer usage skills - acquired through completion of a bachelors degree in a related mental health care field such as Social Work Nursing Psychology or a Rehabilitation Services specialty.
- For internal candidates high school education plus an additional 5 years of relevant experience (2 years of work must be within SPHS) may be considered a substitute for a bachelors degree.
- Position requires knowledge of psychiatric terminology discharge and aftercare planning methods and community referral resources - acquired through six to twelve months of relevant field or work experience in a psychiatric setting.
