Job Description
Description of the role:
University Home Care is seeking a compassionate MSW for our Hospice Home Care division in Livonia MI. This position plays a crucial role in providing support and counseling to patients and their families during challenging times.
Responsibilities:
- Conduct psychosocial assessments and provide counseling services
- Collaborate with interdisciplinary team members to develop care plans
- Advocate for patients needs and preferences
- Coordinate community resources and services
Requirements:
- Must have a Masters degree in Social Work
- Licensed Clinical Social Worker (LCSW) preferred
- Prior experience in hospice or palliative care
- Strong communication and interpersonal skills
