Job Description

Rock Star Rehabilitation Aide needed to work with our seniors in the Physical Therapy Department!

Gary and Mary West PACE is a thriving and fast growing all inclusive day center for the fragile elderly.

We are located in San Marcos CA.

This is a Monday through Friday role at our day center center. 8:00 to 4:30 schedule.

Position Summary:

Under the supervision of Physical and Occupational Therapy The Rehabilitation Aide will provide support to the Rehabilitation team GMWP.

Essential Functions:

  • Assist in scheduling transporting and transferring (in facility) participants as required for Physical and Occupational Therapy services.
  • Assist participants in completing non-skilled/Maintenance/Group therapy under the supervision of Physical and/or Occupational Therapy.
  • Document and communicate any scheduling conflicts with participants and appropriate staff.
  • Distribute schedules and other needed information to the appropriate staff and/or participant.
  • Assist the Physical Therapist and PTA with treatment (including but not limited to ambulation ROM hot/cold packs).
  • Assist the Occupational Therapist and COTA with treatment (including but not limited to ADLs therapeutic exercise/activities)
  • Document patients communication engagement and progress consistently under supervision of Physical and Occupational Therapy.
  • Maintain general upkeep of Rehab Gym (sanitizing appropriate gym equipment placing equipment in designated locations performing linen laundry).
  • Remain updated on participant medical and physical information.
  • Provide clerical support to Rehabilitation Staff as needed.
  • Provide appropriate communication with other members of clinical staff (Including but not limited to (Providers nurses social workers home care staff).
  • Attend staff meetings as directed.

Education and Certification:

  • High school diploma or equivalent required.
  • Certified Nursing Assistant preferred.
  • Current BLS and CPR certification required.

Skills & Experience:

  • Minimum of one (1) year of documented experience working with a frail or elderly population required.
  • One (1) year experience in a medical care setting as an Aide in an allied health field is desirable.
  • Bilingual English/Spanish preferred.
  • Skilled in communicating with participants their families and other staff members; ability to establish a helping trusting relationship with participants.
  • Ability to utilize appropriate safety measures when providing care.
  • Ability to work effectively and harmoniously as part of an interdisciplinary team the elderly providers of services and peers.
  • Well organized dependable flexible and resourceful.
  • Bilingual in English/Spanish preferred.

Medical Clearance:

Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
