*These hours are dependent on the operational requirements of the organization
*Mount Carmel Clinic operates out of multiple locations; you may be required to work in other areas

Position Summary
The Primary Health Nurse Ill provides health education and care within the scope of nursing practice. As a member of a client-centered interdisciplinary team the Primary Health Nurse Ill provides care to individuals promoting wellness prevention of disease and/or disability and the management of chronic diseases/conditions. The incumbent works as an advocate on behalf of the client to ensure the appropriate links/referrals to other human services that would enable the client to achieve their health goals.
Qualifications
Education & Licensing
Must be a member in good standing with CRNM.
Must be licensed to practice in Manitoba
Must have experience taking responsibility for the activities of a nursing unit or program.
BN preferred (RN with appropriate experience may be considered)
CPR certification at the Basic Rescuer Level
Skills Expertise and Experience
A minimum of five (5) years experience working as a Registered Nurse
A minimum of three (3) years experience working in a community health setting Knowledge and Experience working with Students and/or Volunteers
Knowledge or experience in chronic disease management
Current CPR Certification
Proficient in Microsoft Office programs Community EMR/Accuro and eChart

Knowledge and understanding of Indigenous culture and traditions and the impact of colonization on the health and well-being of Indigenous individuals and families

Strong knowledge commitment and demonstrated ability to apply diversity equity cultural safety pro-choice social determinants of health population health harm reduction trauma-informed and community development principles in all aspects of the work
Communication
Excellent verbal and written communication skills and proficiency in the use of computer systems
Assets:
Strong working relationship with Community Resources
Fluency in additional languages relevant to the client population
Certification in the area of specialization. (RNAP Certified Diabetes Educator etc.)
Experience working with service-specific programs including EMR eChart and Accuro
