Opportunity
The qathet Complex Care Housing (CCH) program is a collaboration between LIFT community Services Society (LIFT) Vancouver Coastal Health (VCH) and Tlaamin Nation. This program represents a suite of services and supports aimed at ensuring people with significant mental health substance use and other health complexities attain safe and stable housing as a foundation to thrive. These services support participants through transitions with the appropriate level of service as their needs change over time in line with Tier 1 Intensive Supportive Housing and Tier 2 Stabilization and Respite and represent an increase in services offered at Supportive Housing and select community-based shelter and housing sites.
LIFT is seeking a Clinical Counsellor to join the multi-disciplinary qathet CCH team to collaborate with the healthcare team to support clients development of physical social emotional and life skills.

- Hours: Part time 17.5-20 hours/week
- Shifts: Monday Sunday/8am - 5pm
- Hourly Wage: $43.95 - $48.60/hour

Key Tasks and Competencies:
- In collaboration with client and multidisciplinary team provides clinical biopsychosocial and spiritual assessments to determine clients strength and resources along with mental health and/or substance use concerns.
- Provides individual and group counselling for a variety of disorders including but not limited to substance related disorders trauma and stressor related disorders mood disorders anxiety disorders obsessive compulsive disorders and personality disorders.
- Provides education and support to clients their families and clinical team members that is culturally and linguistically sensitive to the clients needs.
- When appropriate assumes care coordination responsibilities as a part of the interdisciplinary team.
- Provides referrals to inter and intra agency resources as appropriate.
- Participates in interdisciplinary case conferences/team meetings and provides consultation and education to help advance the holistic objectives of client care needs.
- Provides communication to other team members as appropriate regarding patient status/progress through documentation of relevant clinical information referral correspondence client interaction and client care plans in a timely manner.
- Develops positive working relationships/partnerships with other team members agencies and health care professionals in order to promote coordinated service quality care and access to appropriate resources for clients.
- Engages in on-going team planning development evaluation and quality improvement activities that facilitate team goal setting problem solving conflict resolution and collaboration.
- Maintains and updates knowledge by reviewing relevant literature consulting with other disciplines evaluating clinical practice and participating in professional development.
- Collaborates and consults with the interdisciplinary team other health care professionals/partners and clients/families to establish appropriate goals and holistic care plans.
- Promotes interdisciplinary collaboration in the identification and resolution of client care issues treatment concerns continuity of services cultural and language barriers ethical dilemmas crisis management and community resource coordination.
- Ensures a safe environment for clients and staff by adhering to violence prevention infection control occupational health and safety standards.
- Performs housekeeping duties as required checks the operation of relevant equipment to ensure safe working order and reports incident/equipment problems or general safety concerns to Supervisor.
- Attends and participates in staff development programs team meetings and other meetings as required.
- Participates in training and orientation of staff students and client/families as required.
- Carries out responsibilities in accordance with health and safety requirements.
- Performs other related duties as assigned.
Education Experience and Qualifications:
- Masters Degree in a health science discipline (such as counselling psychology social work) from a recognized program that includes a clinical supervised practicum.
- Minimum of five (5) years recent related experience providing care through a holistic mental health substance use and primary care lens.
- A combination of education training and experience may also be accepted.
- Current full registration with applicable regulatory College or eligibility for registration with the appropriate professional body or association as applicable.
- Experience with trauma-informed care and demonstrated ability to practice from a harm reduction approach.
- Valid BC Drivers License and access to personal vehicle for local area travel is required.
Knowledge Skills & Abilities
- Demonstrated ability to promote Indigenous Cultural Safety
- Demonstrated ability to provide effective service in a variety of community settings and with diverse populations especially those with mental health or substance use disorders.
- Demonstrated understanding of Trauma Informed Practice Harm Reduction and Recovery Oriented Care.
- Knowledge of the Contextual Model of Counselling Therapy and/other Common Factors Approach that inform evidence-based counselling.
- Comprehensive knowledge of other health care disciplines and their role in patient/client care.
- Demonstrated ability in dealing with a variety of situations and responsibilities requiring initiative creativity and professional judgment.
- Knowledge of the principles and practices of a client-centered recovery model in mental health.
- Working knowledge of DSM (Diagnostic and Statistical Manual of Mental Disorders).
- Knowledge and skills to provide care coordination to a select caseload of clients.
- Demonstrated knowledge to assess patients/clients at risk and knowledge of appropriate legislation acts e.g. Health Care Consent Act Mental Health Act Adult Guardianship Act Continuing Care Act Freedom of Information and Protection of Privacy Act as it relates to the rights and obligations of clients and staff.
- Demonstrated ability to provide competent and culturally safe care in a variety of settings and with diverse populations.
- Broad knowledge of crisis intervention and foundational counseling skills including Motivational Interviewing.
- Broad knowledge of mental health concerns and treatment.
- Broad knowledge of substance use and addictions treatment.
- Broad knowledge of other facilities and community resources.
- Demonstrated ability to support seamless transition in care and to recognize when referral to another health care professional is required.
- Demonstrated ability to communicate effectively both orally and in writing with patients/clients and their families colleagues Primary Care Providers and other health care staff both one-on-one and in groups.
- Demonstrated listening and information seeking skills that promote communication and lead to a cooperative approach to problem solving within a multidisciplinary setting.
- Demonstrated ability to adjust schedule to deal with unexpected situations.

- Skilled in the use of equipment and in the techniques appropriate for treatment.
- Demonstrated physical ability to perform the duties of the position.
