Case Manager Qualified Mental Health Professional

The Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical developmental intellectual mental health challenges; those who have suffered a trauma; and those at risk to enjoy lives of dignity and purpose. We are looking for aCase Manager Mental Health Professional (MHP or QMHP)who demonstrates this mission and wants to work for an organization that makes a difference in the community.

What you will be doing

  • The residential Case Manager MHP/QMHP is responsible for developing andimplementing individual treatment plans utilizing the IMCANS to maximize the clientsindependent community living skills.
  • The Case Manager MHP/QMHP must complete documentation of mental healthservices provided in accordance with state and agency standards as applicable.
  • The Case Manager MHP/QMHP is expected to work with a team of behavioral healthservice providers within the agency to ensure a positive client-centered approach.
  • This job position may have some work components that can be performed remotely.Remote work arrangements are not a right or entitlement of employment. They arediscretionary and subject to demonstrated performance and operational needs.Approval may be rescinded at any time at the sole discretion of management.

Essential Job Responsibilities

  1. Work scheduled hours and be flexible to meet client and program needs as assigned byProgram Manager or Director.
  2. Responsible to provide independent program coverage when scheduled.
  3. Assure compliance with all agency state and federal regulations while providing servicesand completing assignments. Review and follow updated policies and procedures.
  4. Facilitate communications and coordination of services with other AID staff andprofessionals in the community.
  5. Maintain professionalism and good boundaries when working with clients coworkers andoutside agencies.
  6. Meet minimum service hour provision productivity standards (MRO) monthly.
  7. Complete all required case management documentation (assessments IMCANSconsents residential/agency paperwork) on a timely basis.
  8. Complete and sign all MRO Documentation within 48 hours using Cx360
  9. Meet with all assigned clients on regular basis depending on program and client needs;submit daily activity logs.
  10. Develop review and revise the IMCANS and complete corrections within the timeframeallotted.
  11. Complete assessment tools and necessary paperwork prior to staffings (includingLOCUS SAMA consents client rights and privacy practices) per program procedure.
  12. Develop and provide direct implementation of treatment goals for assigned caseload.
  13. Assist clients with activities of daily living including apartment maintenance and provideclient training consistent with the treatment goals.
  14. Provide guidance to MHP and RSA staff regarding implementation of IMCANSgoals/objectives.
  15. Obtain Input from clients families guardians and other staff on how to improveresidential services.
  16. Monitor and report safety of clients living arrangement
  17. Acquire and maintain required certifications and trainings as well as any other trainingsassigned by Manager.
  18. Assist clients in paying bills budgeting and banking per program procedure.
  19. Oversight or management of client funds when necessary (dual signature accounts AIDrep. Payees).
  20. Obtain and maintain client benefits (Social Security Medicaid Link Card Etc).
  21. Assure client records are properly maintained per agency procedures.
  22. Complete authorizations reauthorizations and spend-down paperwork in a timely fashion.
  23. Complete QI file corrections and submit by deadline given.
  24. Update Cx360 with corrections whenever necessary but at least annually to ensureaccuracy of records.
  25. Provide effective services for clients individual needs and in line with client rights.
  26. Attend monthly clinical supervisions per DHS requirement
  27. Attend monthly team meetings and be a positive contributor.
  28. Recognize emergency situations and take appropriate action. Contact Manager andDirector per procedure. Complete necessary paperwork correctly (incident reports petitionsencounters).
  29. Promote a relationship between clients families and additional supports.
  30. Provide transportation to clients in personal or agency vehicles when necessary for workshopping appointments etc.
  31. Maintain a positive and professional working relationship with coworkers agencies andother stakeholders.
  32. Maintain a team approach to services.
  33. Maintain client rights.
  34. Assure the coordination of services in the community to meet client needs (ie. Financiallegal advocacy medical ect.)
  35. Promote community / family involvement in facility programs.
  36. Participate in and promote organizational and division fund-raising and annual events.
  37. Participate in the planning and implementation of client activities.
  38. Follow program and agency procedures for notifying management when unable toattend a scheduled workday.
  39. Perform all other related duties as assigned and in accordance with AID policies andphilosophies.
  40. Promotes client wellness through coaching counseling consultation motivationalenhancement and information and referral services to clients facing various healthchallenges as needed.
  41. Communicates with service delivery partners which may include Physicians NursePractitioners Counselors Pharmacists Employers Community Supports and otherHealth Professionals as needed to facilitate the achievement of clients wellnessobjectives..
  42. Utilizes a variety of appropriate resources and educational materials for wellnesscoaching. (ie. Health Matters Curriculum Smoking cessation materials WeightManagement information etc.)
  43. Promote awareness and respect for the diversity of our clients employees familiesand other stakeholders.

What will you bring to the table

Education:

  • Bachelors degree in Human Services or related field required
  • Masters degree required (for QMHP)

Experience: Experience working in social services required.

Physical:

  • Navigation of stairs
  • No lifting restrictions.
  • Ability to provide services in clients homes.

Equipment:

  • Computer including Microsoft Windows applications
  • Copy Machine
  • Telephone with voice mail system

Additional Requirements:

  • The use of personal automobile a valid drivers license and the minimum amount of liability insurance as defined by AIDs Personnel Policy. Drive self and clients in agency or personal vehicle.
  • Evening weekend and holiday hours required
  • Coverage at other supervised/24 hour facilities may be required.
  • Must be able to drive a passenger vehicle
