Kyabram District Health Service (KDHS) is a medium-sized rural health service that delivers a range of programs and services including acute subacute residential aged care and allied and primary health care services. KDHS has three campuses located in the townships of Kyabram Stanhope and Tongala.

About the Role
The Clinical Care Coordinator at Sheridan Aged Care is a senior leadership role ideal for an experienced clinician passionate about driving excellence in residential aged care. This role leads the coordination of safe individualised care supports AN-ACC optimisation and ensures compliance with the Aged Care Quality Standards. The Coordinator plays a critical part in clinical governance innovation and continuous improvement working collaboratively with a committed multidisciplinary team. With responsibility for mentoring staff managing complex care needs and influencing quality outcomes this role offers the opportunity to make a meaningful impact in a supportive purpose-driven organisation.

You are positive motivated professional with a passion for aged care and quality of life. You are approachable team orientated and capable of inspiring others through your initiative and creativity. You bring strong communication and customer service skills along with the ability to build meaningful relationships with residents and their families
