About the Role
As a Care Coordinator you will assess eligible high-risk clients living with complex healthcare needs. The primary aim of the position is to provide generic care coordination that facilitates the clients self-management strategies in order to improve health outcomes and prevent avoidable hospital presentations. Intervention is provided through a collaborative care coordination approach and will integrate traditional hospital based health care services and primary health care. This role includes conducting comprehensive assessments coordinating all aspects of health and social care needs monitoring achievement of client goals facilitating intervention from other health care professionals and ensuring appropriate linkages with hospital and community services.
Classifications for this position will be Registered Nurse Grade 4A ($116760.80 per annum).
Your Contribution
Bachelor or Masters Degree in Nursing
Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
Completion of basic life support training/CPR/First Aid
Minimum of 3 years post registration experience.

About the Cottage
This position will sit within the SVHM Healthcare for the Homeless program multi-disciplinary ALERT stream and will provide a proactive service to clients staying at the Francesca Healy Cottage. The key target groups for ALERT include patients with complex needs who fall under one or more of the key risk factors; substance use mental health issues homelessness disability and complex aged care issues family violence. The Cottage is a six bed health facility in a homely terrace house setting in conjunction with Hospital in the Home providing personal and nursing care for homeless or at risk of homelessness clients. The aim is to ensure that that physical social and emotional needs of the patient group are addressed within an environment of safe practice respect for dignity and holistic care.

LOCAL WORK ENVIRONMENT
This position is part of the Health Care for the Homeless (HfH) Program. HfH comprises several services that support clients that are experiencing or at risk of homelessness. HfH aims to integrate and better coordinate existing services within St Vincents Hospital and where service gaps are identified support the development of new services to meet the needs of our clients.
The Assessment Liaison and Early Referral Team (ALERT) and the Cottage sit within the HfH Program. The key target groups of ALERT include patients with complex needs who experience one or more of the following key risk factors: substance use mental health issues homelessness disability complex aged care issues and family violence.
The Cottage is a five bed health facility in a homely terrace house setting providing personal and nursing care for homeless women and men. The aim is to ensure that the physical social and emotional needs of patients are addressed within an environment of safe practice respect dignity and holistic care.
POSITION PURPOSE
As a Care Coordinator you will provide person-centred care coordination for clients living with complex psycho-social and healthcare needs utilising discipline specific skills to support clients to improve their self-management skills. Intervention is provided through a collaborative care coordination approach and will integrate traditional hospital based health care services and primary health care.
You will conduct comprehensive holistic health assessments coordinating all aspects of health and social care needs monitoring achievement of client goals facilitating intervention from other health care professionals and ensuring appropriate linkages with hospital and community services.
Your clinical intervention delivered will be of high quality person-centred and in accordance with relevant codes of practice clinical standards and scope of practice.
