We are seeking a dedicated and detail-oriented Clinical Care Coordinator to join our healthcare team. In this vital role you will be responsible for the coordination of patient care activities to ensure that our patients receive the highest quality of care throughout the healthcare continuum. You will act as a liaison between patients families and healthcare providers facilitating communication and collaboration to improve patient outcomes. Your role will involve assessing patient needs developing care plans and monitoring the progress of patients while ensuring compliance with health standards and regulations. You will use your strong organizational skills and clinical knowledge to manage patient information schedule appointments and follow up on treatment plans. Responsibilities
- Coordinate patient care across various disciplines to optimize treatment and outcomes.
- Develop and implement individualized care plans based on patient assessments and needs.
- Facilitate communication among patients families and healthcare providers to ensure continuity of care.
- Monitor patient progress and maintain up-to-date records in accordance with healthcare regulations.
- Educate patients and families about treatment options medications and care procedures to empower informed decision-making.
- Assist in scheduling appointments referrals and follow-up visits for patients.
- Participate in quality improvement initiatives aimed at enhancing patient care and operational efficiency.
Requirements
- Bachelors degree in nursing public health or a related field.
- 2 years of experience as a Registered Nurse in a home health setting.
- Current nursing license (if applicable) or relevant certification in healthcare coordination.
- Proven experience in a clinical setting preferably in care coordination or management.
- Strong communication and interpersonal skills to effectively engage with diverse patient populations.
- Ability to work collaboratively in a multidisciplinary team environment.
- Proficient in electronic health record (EHR) systems and Microsoft Office Suite.
- Strong organizational and time management skills to handle multiple responsibilities and priorities.
