Primary Purpose:
Work with a defined group of students eligible to receive special educations services (ages 13 or younger if
applicable) to ensure transitional plans are developed and implemented. Responsible forthe transition assessments
interviews goal setting defining coordinated set of activities course ofstudy inter-agency connections family
support for this target group. Provide special education students withlearning activities and experiences designed to
help them fulfill their potential for intellectual emotionalphysical and social growth. Modify curricula and prepare
lessons and other instructional materials to studentability levels. Work in self-contained team departmental or
itinerant capacity as assigned.
Qualifications:
Education/Certification:
Bachelors degree from accredited university
Valid Texas Special Education Teaching Certificate
VAC Certification (preferred)
Special Knowledge/Skills:
Basic understanding of transition law transition service requirements and implementation strategies
General knowledge of transition documentation requirements
General knowledge of work-based learning
Ability to communicate with staff administrators students teachers parents and community partners
Ability to work collaboratively with multi-disciplinary teams
Ability to work collaborative with agency representatives and community partners as part of the coordinated
set of activities requirement in transition services
Attend ARD meetings as appropriate
Knowledge of special needs of students in assigned area
Experience:
At least three years teaching in special education at the secondary level
Secondary experience with the implementation of transition requirements in the secondary teacher role
Some experience in work-based learning and/or Career and Technology Education is preferred
Such alternatives to the above qualifications as the administration may find appropriate and acceptable.
Major Responsibilities and Duties:
Assist the District Transition Specialist to develop implement and monitor a district-wide plan to provide
appropriate and effective transition services.
Conduct informal and formal transition assessments and/or monitor the administration of transition assess-
ments administered by campus staff to identify strengths needs preferences and interests.
Facilitate completion of IEP transition paperwork; including course of study coordinated sets of activities
and post-secondary goals and transition and vocational assessment reports.
Counsels or works with staff who counsel students with disabilities regarding transition assessment results
and post-secondary goals related to education/training employment and independent living if appropriate
Provide consultation to special education staff regarding transition opportunities for students served by
special education
Collaborate with secondary staff in the preparation of the IEP and transition services
Attend ARD/IEP meetings to assist in the development of transition services as needed.
Works collaboratively with agency representatives to provide information sessions for students and parents
conduct eligibility and application activities and work to transition students to public school services and
supports to adult services and supports
Work collaboratively with school counselors to encourage students with disabilities to consider post-
secondary education make selections based on interests and preferences
Provide information and training for students regarding transition and self-determination
Work with campus staff and district administrators to identify parent involvement activities that support
transition for their child
Ensures transition services consider the impact and needs of children with disabilities related to multi-
cultural and diversity issues
Conduct regular evaluations of transition services effectiveness and compliance
Plan and use appropriate instructional and learning strategies activities materials and equipment that
reflect understanding of the learning styles and needs of students assigned
Work cooperatively with classroom teachers to modify regular curricula s needed and assist special education
students in regular classes with assignments
Present subject matter according to guidelines established by IEP
Consult district and outside resource people regarding education social medical and personal needs of
students
Take all necessary and reasonable precautions to protect students equipment materials and facilities
Consult with classroom teachers regarding management of student behavior according to IEP
Work-Based Learning Responsibilities
Work collaboratively with Career and Technology Education to ensure CTE is the first consideration and
least restrictive environment for students with disabilities considering vocational courses and activities
Perform other duties as assigned by the Special Education Director and District Transition Specialist/TED
Other Responsibilities
Participate in staff development activities to improve job-related skills
Keep informed of and comply with federal state distr4ict and school regulations and policies for special
education teachers
Compile maintain and file all physical and computerized reports records and other documents required
Maintain documentation of student contract times
Maintain confidentiality
Supervisory Responsibilities
Supervisor assigned teacher aide(s)

Maintain emotional control under stress. Frequent standing stooping bending pulling and pushing.
