JOB SUMMARY:
The Commonwealth Center for Children and Adolescentsis seeking aTransition Specialistfor the2025-26 School Year. We are seeking a dedicated and compassionate Transition Specialist to join our team at the Commonwealth Center for Children and Adolescents. The Transition Specialist will play a critical role in preparing students for success beyond the hospital setting by providing instruction and support in essential life and vocational skills. This position is ideal for a dynamic educator who is passionate about empowering youth with the tools needed for increased independence self-advocacy and successful community reintegration.
Responsibilities:
- Develop and deliver engaging developmentally appropriate lessons across a range of transition-focused topics including:
- Financial Literacy
- Activities of daily living (ADLs)
- Job readiness and vocational skills
- Self-advocacy and decision-making
- Social and emotional learning (SEL)
- Basic meal preparation and nutrition
- Career awareness and exploration
- Responsible and safe technology use
- Community participation and navigation skills
QUALIFICATIONS/EDUCATIONAND EXPERIENCE:
- Candidates must hold a valid collegiate or postgraduate professional license in Special Education Career and Technical Education or Family Consumer Science.
- Experience working with children and adolescents with emotional and behavioral challenges.
- Knowledge of transition planning IEPs and vocational/independent living skill instruction.
- Strong interpersonal skills flexibility and the ability to work effectively within a team.
- Passion for empowering youth and fostering resilience and independence.
11-month contract (220 days).
