This is a Stanford Health Care job.A Brief OverviewTransfer Center Nurses provide 24-hour coverage in the Transfer Center to facilitate smooth transfer or transport and transition for all emergent urgent and elective patients into the Stanford University Medical Center from referring hospitals clinics and physicians offices ensuring they are provided the appropriate level of care bed financial screening and timely appropriate mode of transport. Transfer Center Nurses evaluate process track document and coordinate patient transfers and patient transports into and out of the Stanford Medical Center. Transfer Center Nurses efficiently engage appropriate departments in the transfer process and are responsible for oversight of the entire transfer episode from start to Clinical Nurse (RN) job classification is represented by the Committee for Recognition of Nursing Achievement (CRONA). CRONA is an independent union for Registered Nurses at Stanford Health Care and Lucile Packard Childrens Health Stanford. What you will do- Contacts Case Manager at insurance or review organization to establish case management.
- Coordinates emergency admission clinical issues as needed.
- Coordinates with staff of other hospital departments including Medical Communications Case Management Social Work patient care units and outpatient clinics to ensure that the patients needs are met from admission through discharge.
- Documents findings and recommendations for accepted transfers or transports.
- Performs nursing assessment to determine the appropriate level of care at the hospital (i.e. critical care acute care observation or outpatient) and the need for specialized equipment isolation or other patient needs; provides nursing assessment information to patient care unit or clinic accepting the transfer or transport and coordinates the arrival time; assesses implements and evaluates appropriate interventions for a patients current or changing clinical status.
- Provides an expert level of clinical decision-making to determine patient acuity need for transfer appropriate means of transfer appropriate level of care during transfer or transport and to sort through all patient information and formulate an accurate picture of the patient and their condition for the hospitals physicians and nurses.
- Provides nursing care to patients with the more acute or complex conditions or in emergencies in accordance with Transfer Center protocols.
- Screens patient for anticipated discharge planning needs such as acceptance of patient back to referring health care institutions skilled nursing facility (SNF) and/or other community resources to ensure patient and family needs are met.
- Serves as an expert resource and consultant to other nurses physicians departments and families of patients.
- Sorts through all patient information and formulates an accurate and complete picture of the patient and the patients condition for the hospitals physicians and nurses; confers with Admitting Physician when questions or issues arise regarding medical necessity of transfer; confers with Social Workers or Case Managers regarding patient and family psychosocial needs.
Education Qualifications- Graduation from an accredited school of nursing
Experience Qualifications- Three (3) years of progressively responsible and directly related work experience
Required Knowledge Skills and Abilities- Knowledge of the Emergency Medical Treatment and Active Labor Act (EMTALA) and related regulations
- Knowledge of the requirements and regulations of third-party payers such as Medi-Cal Medicare CCS and other government programs as well as private insurance
Licenses and Certifications- NursingRN - Registered Nurse - State Licensure And/Or Compact State Licensure .
- BLS - Basic Life Support .

Base Pay Scale: Generally starting at $92.64 - $106.87 per hour
The salary of the finalist selected for this role will be set based on a variety of factors including but not limited to internal equity experience education specialty and training. This pay scale is not a promise of a particular wage.
