Job Summary
The Social Worker for UHNs Blood Disorders Program has a focus on supporting adult outpatients in the Red Blood Cell Disorders (RBCD) clinic.
Duties
- Collaborate with a core interdisciplinary team to support people living with hemoglobinopathies including Thalassemia and Sickle Cell Disease
- Demonstrate an excellent understanding of oppression from an Intersectional lens including but not limited to discrimination racism Anti-Black racism ableism disablement Islamophobia and xenophobia which are frequently encountered by patients living with hemoglobinopathies.
- Demonstrate experience working with these patient communities to address inequities and improve patient experience and outcomes
- Recognizing that patients who access RBCD clinic are from diverse cultural and ethno-racial heritage and socioeconomic circumstances outstanding communication skills and work experience with social justice and health equity frameworks will be important characteristics for the successful candidate
- Assess patients for the purpose of evaluating the need for service the nature of the problem and formulating recommendations or treatment plans; implementing an intervention plan with patients family members significant others and the inter-professional team for the purpose of facilitating changes in behaviour attitudes feelings and/or the environment
- Re-Aeassessing and modifying the intervention plan as required
- Liaising with appropriate community agencies
- Completing applications for services as necessary establishing contact with members of the inter-professional team and community agencies to obtain provide or exchange information relating to patients psycho-social functioning and health care in the past present and future
- Rendering services for the immediate benefit of the Hospital and the community
- Providing clinical teaching to Social Work Students Medical Students Professional Colleagues at University Health Network Faculties of Social Work and in the community
- Participating in relevant research as well as patient engagement and education activities as well as projects etc.
Qualifications :
- Completion of a Masters Degree in Social Work
- Completion of undergraduate education focusing on Social Work Sociology Critical Race Studies Ethnic Studies Disability Studies and Gender Studies preferred
- Registration with the Ontario College of Social Workers and Social Service Workers required. Lived experience as a member of an equity-seeking group including members of Black and Racialized communities preferred
- Skilled in providing short-term counselling and case management to meet the needs of large patient volume living with chronic illness and/or disability. Demonstrated experience in working within strengths-based empowerment and trauma-informed approaches preferred.
- Two to three years of experience providing social work support to clients experiencing poverty racism anti-Black racism immigration and settlement chronic health issues disability violence and abuse or homelessness required
- Strong understanding of and commitment to Anti-Oppression Anti-racism Human Rights and Social Justice
- Excellent advocacy skills in a social justice framework with demonstrated experience supporting clients facing marginalization
- Excellent interpersonal team work time management organizational and problem solving skills
- Research experience/education and student education/supervision an asset
- Knowledge of policies legislation programs and issues related to social welfare institutions and services
- Excellent writing skills
- Knowledge of a second language an asset
