About us

Community Care Tasmania is a not for profit focusing on providing compassionate care and support enabling our clients to thrive at home with dignity. We are seeking a dynamic and dedicated individual to become one of the team in our Launceston Office.

We are expanding our Clinical team and are looking for a fully qualified Enrolled Nurse with a minimum of five years experience in a similar role ideally in a high-care or community complex care environment with current and unrestricted registration with AHPRA and Professional Indemnity Insurance. This role offers a variety of office based and community based duties so that no two days look alike.

Summary:

The Enrolled Nurse will undertake all duties in a caring and supportive manner consistent with CCT core values to provide an exceptional standard of care.

Primary Tasks include:

  • Deliver evidenced based care in accordance with the clinical support plan under the direction and supervision of the registered nurse.
  • Assist in the collection of comprehensive clinical data from the client and/or family.
  • Provide information to the client and/or family in accordance with the clinical support plan.
  • Establish priorities for client care delivery in collaboration with the registered nurse.
  • Promote and participate in education activities including preceptorship and orientation of less experienced staff.
  • Actively participate in quality and safety improvement activities risk management and in the identification and reporting of unsafe practices.
  • Recognise the need for and actively participate in and maintain self/professional development.
  • Actively participate in and contribute to the organisations Quality & Safety and Work Health & Safety processes including in the development and implementation of safety systems improvement initiatives safeguarding practices for vulnerable people and related training.
  • The incumbent can expect to be allocated duties not specifically mentioned in this document that are within the capacity qualifications and experience normally expected from persons occupying positions at this classification level.

About you:

We are looking for someone who is passionate about supporting those in the Aged Care and Disability Communities to stay at home with dignity for as long as they wish. You will be an organised and enthusiastic individual who is eager to be part of a team who enjoy making a difference to others lives.

The qualifications and skills you will bring to the role include:

  • Enrolled Nurse with current and unrestricted registration with AHPRA and Professional Indemnity Insurance.
  • Minimum 5 years experience in a similar role ideally in a high-care or community complex care environment.
  • Strong understanding of person-centred care principles with a focus on promoting individual rights and autonomy.
  • Experience and background in community and/or hospital settings. Palliative care experience is considered a strong advantage.
  • Competent in medication management and wound care.
  • Demonstrate that documentation is completed to an accurate legally and ethically compliant standard.
  • Experience in an office environment with medium to high level knowledge of Microsoft office and data input functionality.
  • Demonstrated contemporary knowledge and skills in the area of medications wound management asthma diabetes and chronic disease management according to qualification competencies.
  • Demonstrated knowledge of person-centered care and goal directed care planning.
  • Demonstrated knowledge and sound client focused care coordination experience.
  • Provide service delivery according to defined targets and to at least the minimum level required by relevant funding streams.
  • Personal passion for community health welfare and embracing a holistic approach to social health and wellbeing outcomes.
  • Excellent organisational skills including the ability to deal with multiple demands and timelines.

You will hold or have the ability to apply for the follow:

  • Police check (less than 3 months)
  • AHPRA registration
  • Current WWVP with NDIS endorsement
  • NDIS Workers Orientation Module.
  • Drivers licence
