Job Description

TEMPORARY UNTIL APPROXIMATELY FEBRUARY 9 2027


Under the direction of the Director Clinical Services the Justice Case Manager provides specialized comprehensive community treatment rehabilitation and support to referred seriously mentally ill individuals in the Windsor-Essex County area who are involved in or at risk of becoming involved in the criminal justice system.


Provides professional case management services within the framework of a client directed clinical/rehabilitation case management model. Provides a continuum of functions including: assertive outreach building therapeutic relationships developing care plans with clients client and systems advocacy symptom management life skills teaching trauma-focused supportive counseling motivational interviewing family support and crisis intervention including wellness checks crisis counselling and risk mitigation plans. Services are delivered to individuals and their families (or their personal support networks).


Justice Case Managers also provide support to the Safe Beds program (i.e. admissions on call coverage monitoring and checking in on temporary residents and providing after-hours and weekend support).

RESPONSIBILITIES & DUTIES:

  • The position is responsible for the following:
  • Assertive outreach: to locate the client after the referral is received. This is integral to dealing with this specialized population. Assertive Outreach involves attending shelters probation appointments street health SWDC and other locations in our efforts to connect with clients.
  • Client orientation. Ensuring clients have an understanding of the services being offered as well as an understanding of processes rights and expectations for participation in the program as well as limits to confidentiality.
  • Identification of client needs through ongoing assessment (OCAN) and treatment/care planning. Initial OCANs are completed within 30 days of admission and a minimum of every six months after that. Assessment will include clinical data from the referral source (Justice Support Services clinicians social worker staff criminal justice system probation and parole police services) to assess individuals level of need risk of recidivism and the overall risk to the community at large.
  • Evaluates clients upon entry to services and throughout service delivery for levels of risk including risk of harm to self and harm to others. Develops risk mitigation plans and strategies to effectively work with each client based on the presenting risk factors.
  • Providing psychoeducation about illness symptom management medication side effects and coping skills. Education includes methods for dealing with conflict anger management reactivity impulse control and other triggering behaviours.
  • Developing delivering and evaluating group educational programs relevant to clients as directed by the manager.
  • Working collaboratively with clients families police COAST/OPP MHRU probation/parole officers courts and other community and justice partners to implement treatment/care plans that focus on strengths wellness and empowerment to support recovery and goal achievement. This may include case conferences or other staff consultations.
  • Provide education to clients to assist with re-integration into the community during and following involvement in the judicial system. Community reintegration may include return to school work and social activities as well as the building of a healthy informal support system.
  • Developing relapse prevention/wellness plans as well as providing crisis intervention.
  • Client and Systems advocacy and linkage to resources: Assists clients to access information as well as internal/external resources services or supports which are deemed necessary. Where resources do not exist or they are inadequate advocates within system to develop or improve essential services and resources for clients.
  • Providing life skill coaching to support activities of daily living and independence.
  • Assisting clients in accessing basic needs such as financial assistance and benefits emergency and/or stable safe affordable housing food banks etc.
  • Engaging educating and supporting families through the treatment and recovery process and encouraging clients to identify and include a care partner.
  • Ensures all file management and reporting functions are maintained in a comprehensive and timely manner.
  • Participates in program evaluation and makes recommendations for changes/improvement.
  • Acts as an Agency Ambassador representing CMHA-WECB in a professional manner in the community and serves on committees work groups or boards as requested and approved by the agency.
  • Participates in Agency Speakers Bureau or other public or targeted educational initiatives as requested by the manager.
  • Provides support and training to volunteers students and other staff assigned to the program area and substitutes for and assists other agency staff where skills are applicable. Assist in evaluation and skill development of students and orientation training of new staff.
  • Ensure up-to-date knowledge of mental health and other relevant legislation including:
    • Developments or trends regarding the courts or Criminal Code of Canada.
    • Ensuring clients have information related to their rights under the Mental Health Act and other related legislation.
    • Ensuring client has awareness and understanding of their rights relating to services duty to warn and confidentiality.
  • Provides on-call support for the Safe Beds Program:
    • Support includes being on-call for admissions and providing check-in and monitoring support to clients using the Safe Beds facility. On-call duties include coverage from 8:30 am until 11:30 pm nightly weekends and holidays. On-call hours and other requirements are detailed in the CUPE Collective Agreement.
    • Safe Beds can be a point of entry to CMHA services as it is utilized by community partners (police TSC ER). Referred individuals may not be known by CMHA and may require additional risk assessment to be completed by staff.
    • On-call duties are provided on a scheduled rotation within the Justice Case Management Team.
    • Providing additional support to Safe Beds On-Call staff as required.

EDUCATION & SKILLS:

  • Knowledge and skill acquired through the completion of Bachelor of Social Work degree (BSW) or Nursing (BScN) from an accredited university is preferred or a minimum of a three (3) year University degree in a field deemed relevant by the employer (i.e. degree in Criminology Psychology Law or other related field) combined with significant case management experience.
  • Registered member in good standing of a professional college deemed relevant by the employer such as the College of Nurses (preferably with Canadian Certification in Psychiatric/Mental Health Nursing) or the Ontario College of Social Workers and Social Service Workers (OCSWSSW) or the College of Psychotherapy in good standing is considered an asset.
  • Comprehensive knowledge of mental illness and treatments; comprehensive knowledge of functions and principles of case management; thorough knowledge of psychotropic drugs; knowledge of relevant mental health policy legislation and regulations; knowledge of community resources; knowledge of the criminal justice system and court processes; proficiency required with MS Windows Office Suite programs i.e. Word Excel Outlook and PowerPoint; proficiency in French language would be considered an asset
  • Computer literacy skills and working knowledge or various Microsoft Windows application is required.
  • Bilingualism in both official languages at the advanced level is considered an asset.
  • A valid Ontario Drivers License with access to a reliable vehicle and proof of current insurance and licensing are required for this position.

Hours of Work: Thirty-Five (35) hour work week. The agencys regular hours of operation are Monday to Friday 8:30 AM to 4:30 PM. Flexible hours are required to meet client and service needs.

Salary range: Grade 8 $35.71 $42.25 per hour

Per 2022 - 2026 Collective Agreement

Applicable shift premiums are paid in accordance with the Collective Agreement Article 17.11.
