Job Description
Job Title: Physical Therapist
Facility: Hillcrest Center
Address: 1245 Church Rd Wyncote PA 19095 United States
Contract: 13 weeks
Shift: 8:00 AM - 4:30 PM
Pay Rate: $64/hr (W2)
Guaranteed Hours: 40 hours per week
Hillcrest Center is seeking a licensed and motivated Physical Therapist to provide high-quality rehabilitative care to residents. The ideal candidate will be experienced in designing and implementing patient-centered therapy plans to promote mobility relieve pain and restore function.
Responsibilities:
- Evaluate patients to determine functional levels and identify rehabilitation goals
- Develop and implement individualized treatment plans
- Use exercises hands-on therapy and equipment to improve patient mobility
- Monitor patient progress and adjust plans as needed
- Educate patients and caregivers on home exercise programs and preventive care
- Maintain accurate and timely documentation of patient care
- Collaborate with interdisciplinary team to ensure comprehensive care
