  • Employment Type: Fixed-term part-time until 1 June 2026 (long service leave cover position with a likelihood to transition to permanent 0.6 EFT following this date).

Your new role:

The Diabetes Nurse Educator will provide diabetes education and advice to clients with diabetes; develop relationships with General Practitioners Endocrinologists and other key stakeholders to enable improved diabetes management; and contribute to meeting the objectives and overall performance within the Early Intervention in Chronic Disease (EICD) funding stream.

The Early Intervention in Chronic Disease (EICD) program is a client focused program that provides opportunities for self-management and coordinated service provision as part of the Prevention and Chronic Illness care service. The PCIC team is comprised of experienced allied health and nursing clinicians working enthusiastically and collaboratively to achieve high quality outcomes for clients.

The aim of the service is to improve client outcomes provide integrated seamless care reduce unnecessary hospital admissions and ensure equitable access to healthcare by providing individualised diabetes management support for patients with diabetes in the community.

The Diabetes Nurse Educator will be responsible for conducting a comprehensive diabetes assessment formulating an appropriate care plan utilising patient-reported outcome measures and providing evidence-based client care. Opportunities are available for coordinating and delivering group-based diabetes programs as well as involvement in our Integrated Diabetes Clinic.

You will be responsible for:

  • Assessing adult clients with a diagnosis of Type 1 or 2 Diabetes Mellitus including clients who are: newly diagnosed managed with lifestyle interventions on oral medications on insulin or other injectable medications.

  • Providing education on diabetes and self-management principles including: self blood glucose monitoring use of oral medication use of insulin and other injectable medications healthy eating principles activity and its effect on blood glucose levels foot care management of hypoglycemia and hyperglycemia and the prevention of/screening for complications

  • Initiating and titrating insulin and other injectable agents in accordance with DNE insulin work instruction

  • Facilitating uptake and adherence of clients with continuous glucose monitoring where applicable

  • Planning and providing group education sessions for diabetes management

  • Working with the multi-disciplinary team to deliver holistic care to clients who have diabetes

  • Engaging with multiple stakeholders including GPs endocrinologists and other health care professionals to ensure clients receive coordinated timely care

  • Involvement in the delivery and evaluation of the Integrated Diabetes Clinic service

  • Actively participating in nursing student supervision and placement planning and coordination and professional development to maintain and enhance clinical knowledge and skills

What you need to succeed:

Registered Nurse

Post Graduate Certificate of Diabetes education credentialled by Australian Diabetes Educators Association (ADEA) or working towards credentialing

Post graduate nursing experience in diabetes management

Demonstrated experience in providing diabetes education and assisting clients in self management of diabetes including familiarisation with the latest technology to support client education

Experience working autonomously and within multi-displinary teams

Experience in Diabetes Nurse Educator clinical service program development planning and evaluation

Strong organisational time management and planning skills

Proficiency with client information management systems Microsoft Office programs and other relevant IT-based platforms
