Roseburg is located on Interstate 5 in Southern Oregons scenic Douglas County. The community is nestled in a lush forested region known as the 100 Valleys of the Umpqua. Responsibilities

Join our compassionate team at Mercy Medical Center and provide vital support to patients and families navigating challenging times.

As a Social Worker youll play a crucial role in our Home Health/Hospice program. Youll conduct psychosocial assessments develop individualized treatment plans and connect patients with essential community resources. Your expertise will empower patients and their loved ones to cope with illness loss and the complexities of healthcare.

What youll do:

  • Conduct comprehensive psychosocial assessments to understand patient and family needs.
  • Develop and implement tailored treatment plans aligned with Medicare and other payer guidelines.
  • Provide short-term therapy crisis intervention and grief counseling.
  • Serve as a vital link to community resources ensuring patients receive the support they need.
  • Collaborate with a multidisciplinary team to provide holistic patient-centered care.

LCSW $38.72 - $58.55; MSW $35.20 - $53.23

Qualifications

What youll need:

  • LCSW or LMSW licensure from the State of Oregon
  • Healthcare experience preferred
  • Masters degree from school of social work accredited by the Council of Social Work Education.
