NON-MANAGEMENT JOB DESCRIPTION
Job Title: Intake and Diversion Coordinator
Exempt/Non-exempt: NON-EXEMPT
Immediate Supervisor: Emergency Shelter Program Manager
Salary Range: $23 - $25
Essential Functions:
Shelter Intake:
1. Primarily responsible for effectively and accurately responding to all housing program inquiries in a timely manner; maintains waitlist tracks and keep records of all program inquiries and referrals and provides monthly reports. As needed and appropriate refers inquiries to other services when not a 360 Youth Services housing program candidate. Required to drive within regional areas for intake assessments.
2. Screen clients for shelter eligibility and special needs
3. Schedule intakes according to level of priority
4. Routinely schedules and completes timely entry in the required databases for the program.
5. Regularly coordinate accurately document and report to Program Managers and housing team regarding program openings and potential clients for these opening. Track and record all required documentation on incoming clients.
Diversion:
1. Assist individuals and families at risk of homelessness find alternative housing and resources before they enter shelters or become unsheltered.
2. Develop creative housing solutions unique to the individual such as family mediation roommate agreements rental agreements repayment agreements verbal and/or written agreements between family members conversations with probation officers landlords etc.
3. Conduct outreach assess needs connect clients with services and develop individualized plans to prevent homelessness.
4. Work to establish housing plans and provide ongoing support to clients.
5. Provide services in a person-centered environment utilizing best practices and methods as directed by the Critical Time Intervention framework.
6. Provide crisis intervention as needed including connecting clients with emergency services and support during challenging situations.
7. Track and document clients progress and outcomes regularly reviewing their service plans and making adjustments as needed.
8. Provide services to clients in non-traditional setting ie. the community home of clients friends/family etc.
9. Perform other duties as assigned.
Knowledge and Ability Requirements:
Knowledge of community resources social services and support programs to make appropriate referrals for
clients. Ability to empower clients to make informed decisions and take ownership of their goals and well-being. Demonstrated skills in strength-based practices detail-oriented strong problem-solving skills and harm reduction.
Must be a positive engaging open to feedback collaborative team player with the ability to work independently and take initiative.
Must have either experience or strong education in providing trauma informed care affirming care for LGBTQ young people and commitment to affirming care with BIPOC populations.
Models adaptability and an awareness of the impact of change. Strong interest in gaining new skills related to trauma-informed care anti-oppressive and intersectional lens.
Education and Experience:
Bachelors degree in social work or related human services field and one-year experience or a minimum of 2 years of experience in social services case management or a related role with a focus on client intake and aftercare services is preferred.
Physical Demands and Other Requirements:
To perform this job successfully an individual must be able to perform the essential duties listed in the job description satisfactorily. While performing the duties of this job the employee is regularly required to talk or hear. The employee is frequently required to sit. The employee is occasionally required to stand; walk; use hands to finger handle or feel objects or controls; reach with hands and arms. Capable of operating standard office equipment including a computer and keyboard calculator typical business machines such as a fax machine and copier. The employee must occasionally lift and/or move up to 20-30 pounds. Specific vision abilities required by this job include close vision distance vision and the ability to adjust focus. The employee is required to have valid drivers license auto insurance and must be able to safely operate a motor vehicle. The work environment characteristics include a noise level which is usually low to moderate.

Reports possible misconduct or relevant errors/ oversight in a timely manner and responsibly addresses related issues with chain of command.
- Communication/ Interpersonal Skills: Consistently conveys clear accurate timely and respectful communication appropriate for the situation. Informs appropriate management staff when incident occurs or a crisis situation. Maintains professional boundaries with clients/residents staff interns volunteers and vendors. Able to successfully share skills and concepts with others and works effectively with others under time and environmental pressures.
- Safety and Agency Resource Utilization: Demonstrates sound judgement and foresight in safety awareness and in daily work. Notifies in a timely manner the supervisory chain of any incidents or circumstances that need attention. Report all suspicions or allegations of abuse immediately to their supervisor. Ensures appropriate utilization of materials and resources and minimizes waste.
