Job Description
Key Responsibilities:
- Perform physical therapy evaluations and develop individualized treatment plans for residents.
- Administer therapeutic interventions to improve residents physical function balance strength and mobility.
- ProvideUltraMIST Therapyfor appropriate wound care cases following facility protocols and manufacturer guidelines.
- Document patient progress treatment plans and outcomes accurately in electronic medical records.
- Collaborate with physicians nurses occupational therapists and speech-language pathologists to ensure comprehensive care.
- Educate residents families and caregivers on therapy plans home programs and safe mobility techniques.
- Participate in care plan meetings team conferences and in-service training sessions.
- Maintain compliance with all federal state and facility regulations.
