KEY ACCOUNTABILITIES:
Applies critical thinking to assess a patients medications for appropriateness and safety.
Utilizes evidence-based practice to ensure expected patient outcomes.
Communicates and collaborates within a multidisciplinary team to progress the plan of care across the care continuum.
Incorporates quality and safety into a patients individualized care plan.
Collaborates with and supports care team in meeting the desired patient and family outcomes by reinforcing education identified in the patients plan of care.
Is expected to maintain specialized knowledge when and if practicing in a unique clinical practice setting such as oncology home infusion and others.
Demonstrates professional and therapeutic relationships with patients families and care team. 8. Participates in quality and process improvement committee work and other projects as assigned to meet the needs of the department and ThedaCare.
QUALIFICATIONS:
Bachelor of Science degree or Doctor of Pharmacy degree from an ACPE accredited school of pharmacy
American Heart Association Healthcare Provider Basic Life Support (BLS) and American Heart Association Advanced Cardiac Life Support (ACLS) except for ThedaCare At Home Oncology and Ambulatory Pharmacists.
Active Wisconsin pharmacist license
WORK ENVIRONMENT:
Climate controlled office setting with daily movement throughout the hospital campus
Ability to move freely (standing stooping walking bending pushing and pulling) and lift up to a maximum of fifty (50) pounds without assistance
Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties Normally works in climate controlled office environment
Frequent sitting with movement throughout office space
Frequent exposure to sharp objects and instruments
Occasional exposure to moving mechanical parts fumes or airborne particles toxic or caustic chemicals and risk of electrical shock.
Possible exposure to communicable diseases hazardous materials and pharmacological agents.
Occasional contact with aggressive and or combative patients.
Use of computers throughout the work day
Frequent use of keyboard with repetitive motion of hands wrists and fingers
Position requires compliance with department specific competencies.
This role will serve the following critical access locations : Berlin New London Shawano Waupaca.
