UHN Connected Care is known for our ability to deliver sustainable change management programs. Born out of a variety of province-wide projects we are focused on the healthcare experience as a whole. UHN Connected Care changes the way that care is delivered and experienced. We do this through driving care integration and digital enablement. Our teams deploy models of care clinical services and digital infrastructure to address key gaps in the health system. Our domains of work include post-acute integrated model of care for short stay patients natural occurring retirement communities (NORCs) outpatient care virtual care Connected Care Hub digital health (eServices and Resource Matching and Referral Rm&R) and Altum Health.

POSITION SUMMARY:

We are seeking a skilled Project Manager to join UHN Connected Cares Toronto Region Delivery Team (TRDT). The project manager is responsible for supporting project activities solving problems and escalating risks for support. This role will support the advancement of digital health technologies that facilitate communication between providers and patients while building efficiencies into day-to-day processes.  Advancement of these technologies will include planning deployment evaluation and sustainment activities as well as leading and supporting engagement with Ontario Health Teams (OHTs) health service providers and reporting accountabilities to our regional and provincial partners. Ultimately the project manager will support the use and adoption of technology solutions aimed at improving patient and clinician experience as well as patient outcomes. Strong organizational and communication skills and technical acumen are essential to coordinate with various stakeholders and drive project success. The successful candidate will be highly effective at planning implementing and delivering projects within an inter-disciplinary team environment.

Duties:

 

  • Establish detailed project plans to articulate time budget resources scope and risks in projects and manage project activities and documentation
  • Regularly engage with project sponsors and stakeholders to collect requirements and to ensure success through project lifecycle
  • Lead highly complex large scope projects throughout project life cycle managing multiple projects and priorities concurrently with the ability to reshuffle priorities as they shift with guidance and collaboration with Senior Program Manager
  • Apply change management and communication strategies to multiple stakeholders and organizations throughout projects
  • Proactively sense make complex workflows/processes/situations. Demonstrate an excellent ability to summarize key information in a succinct way to foster collaboration overcome barriers or address any complications
  • Manage risks by balancing client expectations project scope and taking early action to resolve problems and escalate risks in a timely and concise manner to Senior Program Manager with proposed solutions for discussion
  • Successfully adapt communication style based on stakeholder needs/interests and digital maturity (e.g.  acute care senior leadership primary care networks clinicians Ontario Health colleagues etc.)
  • Develop mentor and coach other team members and provide real-time and formal developmental feedback on an on-going basis
  • Build credibility and foster relationships with peers managers and stakeholders
  • Handle unusually complex problems by breaking them down into their component parts and recommending innovative and creative solutions that will achieve quality and work process improvements and improve future decision making
  • Work in a self-directed manner ensuring successful completion of milestones objectives deliverables and achieving long-term desired results
  • Develop and present material for review and informational purposes using excellent written verbal and visual communication skills with the ability to express complex technical concepts effectively
  • Stay up to date on industry news to support new project proposals

Qualifications :

  • Undergraduate Degree required in Health Informatics Computer Science Business or related field
  • Project Management Professional (PMP) designation and Change Management Certification are assets
  • Minimum 4-5 years Healthcare Project Management experience as a Project Manager preferred
  • Minimum 2 years experience working with digital health tools (e.g. eReferral) EMRs or HIS preferred
  • Comprehensive knowledge of project management (project planning implementation evaluation risk assessment benefits realization) and change management methodologies
  • Knowledge and experience in developing and executing communications and adoption plans (e.g. stakeholder analysis communication strategies and tactics awareness campaigns gathering user feedback)
  • Applicable industry experience in leading projects that support client groups such as Health Care Pharmaceutical Finance Human Resources Education Telecommunications etc.
  • Excellent interpersonal and client relationship skills
  • Supervisory or managerial experience
  • Analytical ability with excellent attention to detail
  • Excellent risk management skills
  • Experience leading vendor management and contract negotiations
  • Strong presentation writing and/or training skills
  • Excellent time-management and organization skills (self-directed)
  • Excellent team building skills listening coaching group facilitation and conflict resolution skills
  • Proficient use of computer software and peripheral equipment i.e. Windows MS Office MS Mail/Outlook MS Project
