Work Model: Hybrid 
Hours: 37.5 hours per week 
Salary: To commensurate with experience and consistent with UHN compensation policy
Status:Temporary Full Time 18 months

Connected Care is a strategic business unit at UHN focused on developing and delivering innovative models of care. Our work focuses on transformative care experiences for patients essential care partners and care provider teams. We break down barriers to change bringing together policy makers funders thought leaders clinicians and patients to the table to build solutions together.

Position Summary

 

Reporting to the Director of Outpatient Strategy the Project Manager will lead the planning and implementation of strategic initiatives that support transformation across UHNs outpatient environment. The work includes high-impact projects focused on enhancing patient caregiver and TeamUHN experiences improving clinic workflows and introducing new models of care.

This work is new and will require ongoing iteration and the ability to work with a team to evolve and refine the future state. You will work closely with clinical and administrative teams patient partners and leadership across UHN. The ideal candidate is a self-starter with a strong grasp of healthcare systems exceptional communication and facilitation skills and demonstrated experience delivering clinical and operational transformation.

Duties:

 

  • Lead planning and implementation of assigned projects ensuring delivery on scope timeline and budget
  • Apply project and change management best practices to guide cross-functional teams through all project phases
  • Handle high complexity problems and provide innovative and creative solutions that will achieve quality and work process improvements.
  • Regularly engage with project sponsors operational leaders and front-line teams to align on needs and priorities
  • Support stakeholder engagement co-design and communication activities across departments and partner organizations
  • Identify and mitigate risks early; escalate critical issues to project sponsors with clear recommendations
  • Develop business cases and project charters to support planning and decision-making
  • Lead project reporting budgeting and tracking activities
  • Facilitate and document process mapping sessions and current/future state design
  • Support procurement vendor management and contract development as needed
  • Translate complex information into clear compelling visuals and presentations for diverse audiences
  • Develop communications engagement and adoption plans (e.g. stakeholder analysis communication strategies and tactics awareness campaigns gathering user feedback etc.) 

Qualifications :

  • 58 years of experience in project management preferably in healthcare environment.
  • Bachelors degree in a relevant field (e.g. Health Informatics Health Administration Engineering Business)
  • Masters degree preferred
  • PMP and/or Change Management Certification preferred
  • Demonstrated success in delivering clinical transformation or digital health implementation projects
  • Experience leading cross-functional teams including both direct and indirect reports.
  • Strong understanding of change management and stakeholder engagement principles
  • Experience developing business cases including cost-benefit and ROI analysis
  • Familiarity with procurement processes and vendor/contract management is an asset
  • Experience with Lean Six Sigma or other quality improvement methodologies considered an asset
  • Proficient in MS Office MS Project Visio and related project tools
  • Exceptional project management and organizational skills with the ability to plan track and deliver complex initiatives
  • Strong analytical and critical thinking skills with the ability to navigate ambiguity and drive clarity
  • Exceptional interpersonal and communication skills including the ability to work with stakeholders at all levels
  • Ability to manage multiple priorities and deliver results in a fast-paced environment
  • Comfortable with uncertainty and able to lead teams through complex change
  • Skilled in facilitation coaching and conflict resolution
  • Committed to equity inclusion and person-centered care
  • Client service orientation with the ability to effectively work with diversity and appreciate that people with different opinions backgrounds and characteristics bring richness to the challenge or situation at hand
