Job Description

A well-established 5-operatory stand-alone surgical practice is seeking a full-time Board Certified Oral Surgeon. The practice is set up with sedation equipment CBCT trained DAANCE-certified assistants and is considered the Center of Excellence for Straumann! This group is offering a sign-on bonus or relocation assistance. Industry-leading base salary plus a percentage of collections. Plus full benefits including 4 weeks PTO 401k/match $5k CE malpractice health insurance and pathway to ownership!   • What is the address of your practice and how long has it been a dental practice Please include all practices.1320 N Post Rd Indianapolis IN 46219
  • How many operatories do you have in each practice5 (4 Equipped)
  • Is your practice digital Charts and X-Rays or just X-RaysCBCT / Partially Digital transitioning to fully digital
  • What types of bells and whistles are in your practice (for example do you have a CEREC or CBCT or scanners or intraoral cameras)We want to work with the new doctor to make sure they have all the updated tools they need to be successful.
  • Do you use a formulary Would the doc be limited in any way for supplies / instrumentsWe do not have a formulary set up yet. We are considered a Center of Excellence for Straumann in our home office.
  • Do you provide GA in practice or hospitalThe doctors take care of their anesthesia. If we need to make accommodations that can be discussed.
  • What is the payor mix in your practice (Are you PPO FFS Medicaid DHMO) What percentage of each payor do you typically see For exampe: we see 50% PPO patients and 50% FFS...30% PPO / Less than 30% Medicaid / 40% FFS
  • In your candidate what types of procedures are you most interested in them having experience with Please list out your current procedure mix: For example we do 40% impacted thirds...20% Extractions / 10% Implants / 15% Diagnostic / 10% Anesthesia / 45% Oral Surgery
  • Whats the first week for a new doctor look like in terms of on-boarding / responsibilities Any training or official onboarding programThe 1st week will be shadowing the current surgeon. Setting up internal paperwork. Learning our systems. Talking with Department heads at Forge to understand how each person can help.
  • What are the days and hours of a typical doctor schedule How many assistants do they get and how many columns are they in Are your Dental Assistants DAANCE certifiedMonday Tuesday & Thursday 8 am - 5 pm / Wednesday & Friday 8 am - 2 pm Some assistants are DAANCE certified but not a requirement.
  • 10. Do you offer a path to ownership / partnership and if so what does that path look likeYes there is a path to ownership that will be offered after the new surgeon has been with the group for a year.
  • How do you compensate your doctors Are there production incentives Whats the average take home annually for your doctors (For example are you giving them a daily rate for a period of time and if so whats the range for that Same with percentage.$400K Base Salary (Negotiable) 35% of Collections activated once the surgeon reaches $1.5 Million in Production (one-time threshold) Hybrid W-2 / 1099 We set a salary (W-2) and true up quarterly (1099) the difference between what has been paid and what is still owed to the surgeon
  • Do you hire as employees or independent contractorsBoth are acceptable for this office
  • Do you have an employment contract and if so is it annual What is your non-compete What is your notice period We do have an employment contract. The non-compete non-solicit is for 2 years and 20 miles.
  • Why is this position open If this is a replacement what was the average monthly production of the doctor being replacedThe current surgeon is getting ready to retire.

  • How many total doctors do you have in the practice Will there be mentorship at all What is the average monthly production of current docs in the practice1. 15 Surgeons within Forge. 2. 1 per practice. 3. Yes mentorship is available from current surgeons.
  • How would you describe the culture of your practice What type of personality do you believe would be the best fit for your culture1. Relaxed hands-off as long as production goals are met. The surgeon has autonomy to control the schedule. 2. Personality; Self Motivated; Self Starter.
