HVCS a Division of Cornerstone Family Healthcare is actively recruiting for a CHI Linkage Specialist to join our growing team in Newburgh NY.


Position Title: Corrections Health Initiative (CHI) Linkage

Department: Community Education & Prevention

Rate of Pay/Salary: $29.68 per hour

Work Location(s): Newburgh / Poughkeepsie

Status: Full Time Non Exempt


General Purpose:

The CHI Linkage Specialist is responsible for day-to-day operations of the CHI Program and for providing Linkage & Navigation and ARTAS services to incarcerated individuals living with HIV and/or HCV within four correctional facilities (Wallkill Shawangunk Green Haven and Fishkill) within NYS DOCCS Green Haven HUB.


Key Competencies:

  • Develop and implement Linkage & Navigation services to all incarcerated individuals living with HIV/HCV.
  • Conduct ARTAS with those not linked to NYS DOCCSmedical care to connect them to medical services.
  • Complete documentation in a clear and comprehensive manner which follows agency standards and requirements.
  • Maintain working relationship with NYS DOCCS staff and follows rules set forth by NYS DOCCS.

Description of Duties:

  • Provide Linkage & Navigation services to all incarcerated individuals living with HIV and/or HCV within NYS DOCCS Green Haven HUB.
  • Provide appropriate service linkages to all clients.
  • Maintain positive working relationship with SURN and all Nurse Administrators within the four correctional facilities.
  • Provide ARTAS to all incarcerated individualsliving with HIV and/or HCV not linked to NYS DOCCS medical care perprogram standards.
  • Develop and maintain resource networks for service coordination.
  • Engage in weekly supervision with the Program Supervisor.
  • Participate in department and program meetings.
  • Hold meetings with facility medical staff and to assess program needs and address concerns.
  • Complete monthly reports and statistics inaccordance with contractual requirements.
  • Collect and enter data into the AIRS database
  • Comply with all NYS DOCCS and facility rulesand regulations and maintain working relationships with NYS correctionalstaff.
  • Maintain extensive knowledge of HIV and HCVrelated information.
  • Attend and participate in monthly department All Staff and other required meetings.
  • Be familiar with Cornerstone policies and procedures and the Employee Handbook.
  • Maintain confidentiality of all aspects of Cornerstone including but not limited to patient confidentiality financials and employee relations.
  • Perform other related duties as assigned.
Requirements
  • Associates Degree or Bachelors Degree in Education Social Work or related field; with 1 year of experience workingwith I/I population knowledge of HIV/HCV; or3-5 years experience in Education Human Services or related field; extensive knowledge of HIV/AIDS experience in correctional systems.
  • Bilingual (English/Spanish) preferred.
