The Division of Geriatrics seeks to recruit a full-time Clinical Social Worker III (CSW) for Dementia Care Aware San Francisco (DCA SF) a Centers for Medicare & Medicaid Services (CMS) innovation model called GUIDE (Guiding an Improved Dementia Experience) starting at San Francisco Health Networks (SFHN) Department of Geriatric-Neurology Cognitive Clinic (GNCC). GUIDE Model program is an 8-year CMS program that will allow SFHN to provide coordinated dementia care to Medicare patients with dementia. Under general supervision of the Medical Director the CSW will work closely with an interdisciplinary team including Neurologists and Clinical Patient Navigator/unlicensed Social Worker on this new program focused on improving the quality of life for people with dementia reducing strain on their unpaid caregivers and supporting people with dementia to remain in their homes and communities. This position supports the administrative and clinical responsibilities of a specialized social service program. On this team the CSW will support the interdisciplinary team to complete patient and caregiver assessments patient alignment care planning home safety assessment ongoing monitoring and supports care coordination and transitional care management referral and coordination of services and supports caregiver education and support and monitoring respite services. The role includes patient evaluation and care as well as program management and evaluation functions.
Specific duties include: conduct initial and annual psychosocial brain health assessment; develop a comprehensive person-centered care plan; serve as primary point of contact for caseload; provide on going monitoring and support for patient and caregiver that maintains a minimum contact frequency to adhere to program requirements; coordinate care and support patients during times of tranition; maintain and manage inventory of local/community services; refer and connect patients and caregivers to community-based services and supports; support and manage outreach and enrollment efforts to enhance program growth; administer a caregiver support training; refer and coordinate in-home respite care; manage and monitor patient alignment through CMS online portal; attend interdisciplinary team meetings; develop and implement tools for clinical care of patients with dementia; collect and collate metrics related to program outcomes and measurements for quality improvement; manage coorespondance with SFHN partner departments and CMS to adhere to all program deliveriables; meet with billing and claims department to ensure accurate billing practices to optimize revenue generation; and help to conduct staff / team meetings and trainings related to dementia assessment and care with regard to social service and behavioral needs.
The CSW will provide direction and guidance to Clinical Patient Navigator/unlicensed Social Worker who preform a full range of psychosocial clinical duties. Clinical care will be provided in-person (clinic and in the field) as well as remotely. Some patients also experience chronic illness have substance use disorders have had severe trauma lack a social support system and struggle to engage in care. Many patients do not speak English as their primary language. Professional development opportunities such as training and education will be available to support this position. This is a position that is funded through June 30 2026 for a rapid implementation of an exciting new program with a dynamic and highly motivated team members who are collectively dedicated to improving clinical care for people living with dementia.
The final salary and offer components are subject to additional approvals based on UC policy.
Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union placement within the salary range will be guided by the rules in the collective bargaining agreement.
The salary range for this position is $41.79 - $72.17 (Hourly Rate).
