Hope For Youths Residential Services Department provides community based trauma informed youth and family driven care to adolescents and young adults in Nassau and Suffolk County. Serving youth from varying points of entry with the child welfare system juvenile justice system and runaway and homeless youth/young adults the opportunities to alter the trajectory for youth in need are abundant. Youth are supported 24/7 by a team of multi-disciplinary professionals paraprofessionals and agency administration that consistently meets standards for accreditation and licensing regulations.
Hope For Youths residential caseworker is a vital team member for the Adolescent Diagnostic and Respite Services program. Supporting youth involved with the child welfare system juvenile justice system and respite bed network the caseworker meets youth needs and build strengths utilizing trauma informed Therapeutic Crisis Intervention and motivational interviewing skills. Responsibilities include but are not limited to completing psychosocial assessments as part of comprehensive diagnostic evaluations gathering documentation creating youth and family driven service plans community-based work (home visits) keeping connection to family and resources supporting youth during crisis and participating in treatment team conferences. This opportunity is being offered as Full-Time 35hrs/ week Monday-Friday 9am-5pm with flexible late days
